Ⅰ类切口手术抗菌药物预防性使用干预方法与效果探讨

摘　　要：目的探讨临床药师在Ⅰ类切口手术抗菌药物预防性使用中的药学服务方法和效果。方法选取实施干预前的2008年6—12月(A组)、行政干预后的2011年6—12月(B组)以及药学服务干预后的2013年1—6月(C组)所有Ⅰ类切口手术出院病历,进行干预前、中、后抗菌药物预防使用合理性对照研究。结果相比与A、B组,C组围术期抗菌药物使用率由99.54%、74.88%下降至55.66%;术前给药时机合理率由35.50%、56.51%上升至88.83%;术后用药疗程小于24 h的比例由50.41%、82.64%上升至93.51%;卫生部推荐预防用药品种使用比例由53.13%、13.55%上升至97.32%,差异均具有统计学意义(P<0.05)。结论临床药师的药学服务模式可行、有效,能提高相关制度的落实和抗菌药物使用规范性。Abstract： Objective To study the pharmaceutical care intervened by the clinical pharmacist on the prophylactic usage of antibactefials in type I incision operation. Methods We selected all cases of type I incision operation which were discharged into pre-intervcation from June to December 2008 （Group A） , administrative interventions from June to December 2011 （Group B） , the clinical pharmacist pharmacy service from January to June 2013 （ Group C） ,to compare pre-intervention with the two phases of post-intervention to promote reasonable use of antibacterials. Results Compared with group A and group B, rate of alltlmicrobial prophylaxis of group C decreased from 99.54% and 74.88% before intervention to 55.66% after intervention;the reasonable rate of the medication occasion increased from 35.50% and 56.51% to 88.83% ;the reasonable rate of post-operative medication timing increased from 50.41% ,82.64% to 93.51%. The use proportion of the antibacteri＇,ds recommended by Ministry of Health was increased from 53.13%, 13.55% to 97. 32%. The differences were statistically significant （ P 〈 0.05 ）. Conclusions The pharmaceutical care model provided by clinical pharmacist is feasible and effective and can significantly improve the implementation of the institution and standardize the process of the use of antibacterials.
