握力训练在乳腺癌PICC置管患者中的应用效果

摘　　要：目的探讨握力训练在乳腺癌PICC置管患者中的应用效果。方法采用便利抽样法选择2012年2月至2013年3月青田人民医院收治的160例乳腺癌PICC置管患者,采用随机数字表法分为观察组和对照组各80例。对照组接受常规处理及指导;观察组在此基础上,每天增加10min握力训练,包括握拳、翘掌、虎掌、转腕及摆腕等5组动作。比较两组患者带管期间的并发症发生率、费用情况及带管舒适度。结果 (1)观察组患者静脉炎发生率(6.25%)低于对照组(20.0%),且严重程度也轻于对照组;观察组上肢肿胀及血栓形成的发生率低于对照组,差异均有统计学意义(均P<0.01);(2)观察组患者人均置管费用、并发症处理所致医疗耗材费用、置管期间总费用均低于对照组,平均带管时间长于对照组,差异均有统计学意义(均P<0.01);(3)观察组患者总体舒适度明显高于对照组,差异有统计学意义(均P<0.01)。结论握力训练可以有效预防乳腺癌患者的PICC置管并发症,减少医疗损耗,提高患者的带管舒适度。Objective To explore the application effect of grip training in breast cancer patients with PICC treatment.Methods By convenience sampling,160patients with breast cancer were selected and equally divided into two groups,80cases in each group.The control group was given routine treatment and guidance,while the observation group applied with 10min-grip training,including fist,palm,wrist and put up the wrist and so on.The incidence of the complication,expenses and comfort with PICC were compared between two groups of patients.Results（1）The incidence of phlebitis was 6.25% in the observation group which was significantly lower than which in control group 20%,and it was less severe than control group.The incidence of upper limb swelling and thrombosis was also lower than control group（P〈0.01）.（2）The cost per person,the equipment consumption caused by complication,and total cost in observation group were less than control group,and the average indwelling time was longer than control group（P〈 0.01）;（3）The overall comfort level in observation group was significantly higher than control group（P 〈0.01）.Conclusion Grip training can effectively prevent complications of PICC,reduce medical loss and improve the comfort degree of patients with tube.
