蝮蛇咬伤致器官功能障碍145例临床分析

摘　　要：目的:探讨蝮蛇咬伤患者器官功能障碍的特点,分析其临床意义。方法:收集145例蝮蛇咬伤患者的临床资料,按就诊时间的不同分为三组,A组(就诊时间≤12 h),B组(12 h<就诊时间≤24 h)和C组(就诊时间>24 h),统计毒蛇咬伤中毒患者的多器官功能障碍发生率。结果:145例蝮蛇咬伤患者器官功能障碍占58.6%,器官功能障碍中以心脏的器官功能障碍率最高,为45.5%(66例),其他肝脏、血液、肾脏和肺的器官功能障碍率分别为25.5%、15.2%、12.4%和2.8%。而多器官功能障碍的发生率A组为37.9%,低于B组的62.5%和C组的84.6%,A组与B、C组比较差异有统计学意义(P<0.05/3)。结论:蝮蛇咬伤中毒可导致多种器官功能障碍,并以心脏受损最为多见,病情进展快。因此,蝮蛇咬伤后应尽快进行救治,尽早保护心脏功能,预防并治疗各器官功能受损。Objective：To guide the clinic treatment through analyzing the clinical characteristics of organ dysfunction caused by Agkistrodonhalys bite. Method：This study retrospectively analyzed all the clinical materials of 145 Agkistrodonhalys bite cases and computed their organ dysfunction rates.The 145 cases were divided into three groups：group A（visiting time≤12 h）,group B（12 h〈visiting time≤24 h） and group C（visiting time〉24 h）,among which the organ dysfunction rates were compared.Result：The result showed that the organ dysfunction rate was 58.6%,of which the cardio ranks the highest（45.5%,66 cases）, the others were that of liver,blood,kidney and lung pancreas,ranking rate respectively 25.5%,15.2%,12.4%and 2.8%.The multiple organ dysfunction rate of group A was 37.9%,group B was 62.5%,grouip C was 84.6%. The multiple organ dysfunction rate of group A was lower than those of group B and group C,which had the statistical significance（P〈0.05/3）.Conclusion：The cardio is prone to be injured in Agkistrodonhalys bite,and preventive treatment of organ dysfunction should be done timely.
