摘 要:目的:比较空气与纯氧复苏对新生儿窒息的疗效。方法:将2012年6月1日~2013年1月31日在台州市第一人民医院产科出生的孕周≥37周的60例窒息新生儿按复苏时给氧浓度的不同分为纯氧复苏组和空气复苏组各30例,记录并比较两组新生儿首次啼哭时间,出生后1 min、1.5 min心率,1 min、2 min的氧饱和度(SpO2),1 min、5 min、10 min Apgar评分,缺血缺氧性脑病发病率,1周内的病死率。结果:空气复苏与纯氧复苏组比较,新生儿出生后1min的心率、氧饱和度及1、5、10 min Apgar评分差异无统计学意义(P〉0.05);但与纯氧复苏组比较,空气复苏组新生儿出现首次啼哭的时间较短。1.5min心率、2 min的血氧饱和度(SpO2)较纯氧复苏组高(P〈0.05)。结论:应用空气或纯氧进行新生儿窒息复苏疗效相似,空气复苏是安全有效的,可以避免纯氧复苏带来的长期有害效应。Objective: To compare the therapeutic effects of pure oxygen resuscitation and air resuscitation in treatment of neonatal asphyxia. Methods: Sixty neonates ( ≥37 gestational weeks) with neonatal asphyxia born in the hospital from June 1,2012 to January 31, 2013 were divided into pure oxygen resuscitation group and air resuscitation group according to the oxygen concentration given during resuscitation, 30 neonates in each group; the first crying times, heart rates at 1 and 1.5 minutes after birth, the values of oxygen saturation ( SpO2 ) at 1 and 2 minutes after birth, Apgar scores at 1,5 and l0 minutes after birth, the incidence rates of hypoxic - ischemic encephalopathy (HIE) and one -week fatality rates in the two groups were compared. Results: There was no statistically significant difference in heart rate and SpO2 at one minute after birth, Apgar scores at 1,5 and 10 minutes after birth between the two groups ( P 〉 0. 05 ) ; compared with pure oxygen resuscitation group, the first crying time in air resuscitation group was shorter; heart rate at 1.5 minutes after birth and SpO2 at 2 minutes after birth in air resuscitation group were statistically significantly higher than those in pure oxygen resuscitation group (P 〈 0. 05 ) . Conclusion: The therapeutic effects of air resuscitation in treatment of neonatal asphyxia is similar to pure oxygen resuscitation. Air resuscitation is safe and effective, which can avoid the long - term harmful effects of pure oxygen resuscitation.
