摘　　要：目的：探讨H型高血压对冠心病患者左心室收缩功能的影响。方法选取2011年12月至2013年6月郑州大学第一附属医院心内科收治高血压合并冠心病患者为研究对象，将患者按照血同型半胱氨酸水平分为H型高血压组和单纯性高血压组，比较H型高血压组与单纯性高血压组中左心室收缩功能不全患者的比例及血脂、脑钠肽（BNP）情况。结果 H型高血压组左室收缩功能障碍患者的比例显著高于单纯高血压组（P＝0.002）；在Logistic回归分析中，H型高血压是左室收缩功能障碍的相关危险因素。结论 H型高血压与冠心病患者左室收缩功能障碍具有明显相关性，治疗冠心病患者时，常规控制血压的同时，降低同型半胱氨酸水平，可能会更有效降低冠心病患者心功能不全发生率，改善预后。Objective To investigate the effect of H type hypertension on left ventricular systolic function in patients with coronary heart disease. Methods One hundred patients with coronary heart disease complicated with hypertension admitted and treated in the first affiliated hospital of Zhengzhou university from December 2011 to June 2013 were selected. According to different levels of homocysteine, they were divided into the H type hypertension group and the simple hypertension group. The proportion of patients with left ventricular systolic dysfunction and the levels of blood lipid,BNP of the two groups were compared. Results In H type hypertension group,the proportion of left ventricular dysfunction patients was significantly higher than that in simple hypertension group（ P=0 . 002 ）;In the Logistic regression analysis,H type hypertension was a risk factor of reduced left ventricular systolic function. Conclusions H type hypertension is obviously correlated with coronary heart disease patients with reduced left ventricular systolic function. In the treatment of coronary heart disease patients,at the same time of routine blood pressure control,reducing the level of Hcy,may be more effective on reducing the incidence of cardiac dysfunction in patients with coronary heart disease and improve their life quality.
