摘　　要：目的了解军队离休干部患者住院费用消耗情况的结构特征,为改进其医疗保障效能提供参考。方法对军队某三级甲等医院2009—2012年出院的军队离休干部患者2 593人次的住院信息进行统计,分析住院疾病费用结构,与师以上干部和地方离休干部患者的住院费用进行对比分析。结果军队离休干部患者住院费用高于其他师以上干部,与同期入院的地方离休干部患者相比,其平均住院日高,但人均住院费用与日均住院费用低于地方离休干部患者。同时,军队离休干部患者住院费用呈逐年上升趋势,呼吸系统疾病消耗费用较多,药费占比较高。结论通过提高诊疗水平、合理用药、减少无效住院日等手段实现对医疗卫生费用的合理使用。Objective To understand the structural features in hospitalization expenses of retired veteran cadres,with an attempt to provide reference for the efficacy,improvement of corresponding health security system.Methods We conduct a statistical analysis on the hospital information of all retired veteran cadres discharged from the three level of first-class hospital during 2009-2012,including 2593 cases.A comparative study was conducted on the hospital cost structure of common disease among retired veteran cadres,cadres including and above the level of division and local retired cadres.Results The total hospital expense of retired veteran cadres was higher than other cadres including and above division level.Compared with that of local retired cadres with the same period of hospitalization,the average length of stay（ALOS） was higher,but the average hospitalization cost of each inpatient and the average cost of each hospitalization day was lower in retired veteran cadres.Meanwhile,the analysis showed an upward trend year by year of hospitalization expense of retired veteran cadres and among the consumption,cost of respiratory disease took a great proportion,so was the medication cost.Conclusion Rational use and scientific management of the health expenses could be achieved by improving the level of diagnosis and treatment,rational use of drug,reducing unnecessary hospitalization days.
