输卵管插管及支架植入术在输卵管手术中的应用

摘　　要：目的对输卵管整形手术或结扎患者行输卵管再吻合术中,宫腔镜联合使用下经宫角输卵管开口插入支架(硬膜外麻醉管),提高术后输卵管的通畅率及术后妊娠率。方法输卵管整形或吻合手术患者76例,36例观察组术中联合宫腔镜行输卵管插管技术,自输卵管开口插入支架穿过输卵管狭窄部或端端吻合处,越过伞端置于腹腔,尾端留于阴道,术后10～15d取出;40例对照组仅于手术疏通后关腹。结果观察组36例65条输卵管,术后2～3个月复查59条输卵管通畅,占90.8%,随访至今有18例妊娠,占50%;对照组40例77条输卵管,术后2～3个月复查31条输卵管通畅,占40.3%,随访至今仅有7例妊娠,占17.5%。结论输卵管整形手术或结扎患者行输卵管再吻合术中输卵管植入支架,能有效防止术后输卵管腔再粘连堵塞和疤痕挛缩,提高妊娠率。Objective TO fallopian tube plastic surgery or tubal ligation patientes another time be identical surgery, hysteroscopy unite apply go through inthmus uteri beginning intercalate brace （hard membrane crust anaesthesia pipe line）, raises after surgery, fallopian tube unobstructed and after surgery pregnant incidences. Methods fallopian tube plastic surgery and be identical surgery 76 cases, 36 cases in observe group to hysteroscopy fallopian tube intercalate plastic surgery, from fallopian tube beginning intercalate braces go through narrow place or end point ang end point place,outstrip umbrella place put inabdominal cavity,afferbody remain in vagina,after surgery 10-15 dayes take out; 40 cases incontrol group to dredge a.river etc shutoff abdominal cavity. Results 36 cases in observe group 65 long narrow pieceis fallopian tube, 2-3 the month after surgery reexamine 59 long narrow pieeris fallopian tube unobstructed, occupy 90.8%, be in company withup to now 18 cases pergnant,occupy 50.0%; 40 cases incontrol group 77 long narrow pieceis fallopian tube, 2-3 the month after surgery reexamine 31long narrow piecris fallopian tube unobstructed, occupy40.3%, be in company withup to now 7 cases pergnant, occupy 17.5%. Conclusion in fallopian tube plastic surgery or tubal ligation patientes another time be identical surgery, fallopian tube intercalate brace, ability go into effect prevent after surgery fallopian tube another time cohere stopup and scarred uterus atrophicraise pregnant incidence.
