复杂肱骨近端骨折采用人工肩关节置换术治疗的远期疗效与并发症探析

摘　　要：目的探讨人工肩关节置换术对复杂肱骨近端骨折患者远期疗效与并发症的影响。方法选取2010年3月-2013年3月期间,我院收治的86例复杂肱骨近端骨折患者作为临床研究对象,将患者随机分为研究组和对照组,各43例。研究组采用人工肩关节置换术,对照组采用锁定钢板内固定术,并分别对两组患者的远期治疗情况和并发症发生情况进行比较和分析。结果与对照组相比,研究组远期治疗效果为优的比率和总优良率均显著提高,分别高达60.47%和93.02%;而为差的比率则显著降低,仅为2.33%,差别均具有统计学意义(P<0.05);研究组并发症总发生率显著降低,仅为6.98%,差别具有统计学意义(P<0.05)。讨论人工肩关节置换术治疗复杂肱骨近端骨折具有远期疗效好,并发症少等优点,可作为复杂肱骨近端骨折患者的首先手术疗法。[ Objective ] To explore the effect of long-term efficacy and complications of arthroplasty treated for complex proximal humerus fractures. [ Methods ] Select from March 2010 to March 2013, 86 complex proximal humerus fracture patients in our hospital. The patients were divided into study group and control group randomly. There were 43 cases in each group. The patients in study group were treated with arthroplasty. The patients in control group were treated with locking plate fixation. The long-term efficacy and complications of the patients in two groups were compared and analyzed. [ Results ] Compared with the control group, the patients in study group, the ratio of excellent and overall excellent for long-term therapeutic effect were increased significantly, up to 60.47% and 93.02%, and the ratio of poor for long-term therapeutic effect was decreased significantly, only 2.33%, there were statistically significant differences （P〈 0.05）. Compared with the control group, the patients in study group, the overall incidence of complications was decreased significantly, only 6.98%, there were statistically significant differences （P〈 0.05）. [ Conclusion ] Arthroplasty has the advantages of better long-term efficacy and fewer complications for the patients with complex proximal humerus fractures. It could be as the preferred therapy of choice for the patients with complex proximal humerus fractures.
