超声对胃癌根治术后胆囊结石症的评估

摘　　要：目的探讨超声评估胃癌根治术后胆囊结石症发病情况的临床价值。方法对胃癌根治术前无胆囊结石、术后存活5年的90例患者（手术组）行定期超声检查，观察胆囊内有无结石、结石发生的时间段及结石有无进行性增大、增多；100例体检健康者为对照组，结果进行对比分析。结果5年内胆囊结石检出率手术组为26．7％（24／90），对照组为10．0％（10／100），差异有统计学意义（P〈0．05）；其中Billroth I式术后胆囊结石检出率10．0％（2／20），Billroth Ⅱ式术后胆囊结石检出率31．4％（22/70），差异有统计学意义（P〈0．05）。手术组结石发生主要集中在术后6～24个月，占75％（18／24）；而对照组发现结石时间为第24—60个月，第36—48个月占7／10。手术组胆囊结石随时间增大、增多、胆囊容积增大的比例均高于对照组（P〈0．05）。结论超声可有效评估胃癌根治术后胆囊结石的发生、发展，为临床提供重要信息。Objective To explore the value of ultrasonography in the evaluation of cholecystolithiasis incidence after radical gastric carcinoma resection. Methods Ultrasonography was performed at regular intervals in 90 patients（operation group ） who underwent radical gastric carcinoma resection and survived more than 5 years. No cholecystolithiasis was found before the operastion. One hundred healthy volunteers （control group） were enrolled at the same time. Results The detection rate of choleeystolithiasis in the operation group was 26.7% （24/90） within 5 years,which was higher than that in the control group （10%）. Significant difference was found between two groups （P 〈 0.05 ）. For the patients received Billroth I and I1 operation, the cholecystolithiasis incidence rates were 10% and 31.4% respectively with significant differenee（P 〈 0.05 ）. Cholecystolithiasis was mainly found from 6 to 24 months after operation accounting for 75%（18/24）. The size and number of gallstone and the gallbladder volume in the operation group were bigger compared with those in the control group （P 〈 0.05）. Conclusion Ultrasound can effectively evaluate the cholecystolithiasis after radical gastric carcinoma resection, which can provides important information for clinic.
