摘 要:目的观察过氧化物酶体增殖物激活受体-γ(PPAR-γ)激动剂罗格列酮对糖调节受损(IGR)患者红细胞聚集性的影响,并探讨其作用机制。方法选取IGR患者90例,按随机数字表方法随机分为对照组45例和实验组45例。对照组给予生活方式干预,实验组给予生活方式联合罗格列酮4 mg/d干预,疗程6个月。治疗前及治疗后第3,6个月测糖化血红蛋白(HbA1 c)、红细胞聚集指数(EAI)、血清可溶性细胞间黏附分子-1(sICAM-1)。结果治疗前2组HbA1C、sICAM-1及EAI均无显著性差异(P均>0.05);治疗后2组HbA1 c、EAI及血清sICAM-1均显著下降(P均<0.01);且实验组HbA1 c、EAI及血清sICAM-1显著低于对照组(P均<0.01);治疗后6个月实验组EAI和血清sICAM-1与3个月时比较均显著降低(P均<0.01),2组HbA1 c及对照组EAI、血清sICAM-1与3个月时比较无显著性差异(P均>0.05);sICAM-1与EAI和HbA1C均呈正相关(P<0.05和0.01)。结论 PPAR-γ激动剂可通过降低IGR患者血清sICAM-1水平,改善其红细胞集聚性,预防糖尿病血管并发症的发生、发展。Objective It is to observe the influence of peroxisome proliferator-activated receptors-γ( PPAR-γ)agonist rosiglitazone on erythrocyte aggregation in patients with impaired glucose regulation( IGR)in order to explore its mechanism. Methods 90 patients with IGR were randomly divided into control group(n=45)and experimental group(n=45). The pa-tients were treated with lifestyle interventions in control group while lifestyle interventions combined with rosiglitazone(4 mg/d)in experimental group for 6 months. Hemoglobin A1c(HbA1c),erythrocyte aggregation index(EAI)and serum soluble intercellular adhesion molecules-1(sICAM -1)were measured before and 3,6 months after treatment. Results Before treat-ment,there were no significant difference between two groups in HbA1c,EAI and serum sICAM -1(P 〉0. 05). After the treatment,HbA1c,EAI and serum sICAM -1 in two group were significantly reduced(P〈0. 01)while those in experimental group were significantly lower than control group(P〈0. 01). HbA1c,EAI and serum sICAM -1 in experimental group at the 6th month were significantly lower than those at the 3rd after treatment(P〈0. 01),however HbA1c in two group and EAI and serum sICAM -1 in control group had no significant changes(P〉0. 05). The level of serum sICAM -1 was positively corre-lated with HbA1c and EAI(P〈0. 05 and 0. 01). Conclusion The level of serum sICAM -1 can be reduced by PPAR-γ ag-onist in order to improve erythrocyte aggregation in patients with IGR,which can prevent the development of diabetes complica-tions.
