补脾益气凉血活血方与激素治疗紫癜性肾炎的临床疗效对比

摘　　要：目的探讨补脾益气凉血活血方与激素治疗紫癜性肾炎的临床疗效比较。方法选取2010年2月至2012年3月于赵县中医院内二科住院的84例紫癜肾炎患者按照简单随机化原则分成两组,中药制剂组44例和激素治疗组40例。中药制剂组以补脾益气凉血活血中药汤剂,激素治疗组给予泼尼松龙,两组均在治疗1周后评价记录临床疗效,治疗2周后分析记录尿常规及尿蛋白定量。结果中药制剂组的临床愈显率为81.8%,总有效率为95.5%,优于激素治疗组的75%和87.5%,差异有统计学意义(P<0.05)。中药制剂组4周后血尿转阴率为100%,优于激素治疗组的35%,差异有统计学意义(P<0.05)。中药制剂组患者在治疗后2、3、4周的蛋白尿显著低于激素治疗组,差异有统计学意义(P<0.01)。结论补脾益气凉血活血方治疗紫癜型肾炎比单纯激素治疗效果显著,尚未出现不良反应。Objective To compare clinical efficacy of spleen invigorating Qi supplementing blood cooling and activating prescription and hormone therapy for purpura nephritis. Methods A total of 84 cases of purura nephritis from Internal Medicine Department Two of Zhaoxian Hospital of Traditional Chinese Medicine during Feb. 2010 and Mar. 2012 were divided into 2 groups at random：44patients,as traditional Chinese medicine group; 40 patients, as glucocorticoid group. The traditional Chinese medicine group was treated with decoction （spleen invigorating Qi supplementing blood cooling and activating prescription）. The glucocorticoid group was treated with oral administration of metacortandralone. The clinical efficacy of the two groups after a week of treat- ment was recorded. The parameters including routine urine test and urine protein excretion after two weeks of treatment were analyzed and recorded. Results The clinical curative rate and total efficiency were 81.8% and 95.5% in traditional Chinese medicine group respectively. The corresponding parameters were 75% and 87.5 % in glucocorticoid group. Obviously, the clinical curative effect of traditional Chinese medicine group was better than the glucocorticoid group （ P 〈 0.05 ）. Hematuria turning negative rate 4 weeks later of the traditional Chinese medicine group was 100%, significantly better than the glucocorticoid group （P 〈 0.05 ）. Proteinuria 2, 3,4 weeks after treatment of the former group were significantly lower than the latter group（P 〈0.01 ）. Conclusion Spleen invigorating Qi supplementing blood cooling and activating prescription decoction of traditional Chinese medicine has a more significant curative effect in treating purpura nephritis than single hormone therapy, with no adverse reaction so far.
