电视腹腔镜手术治疗子宫内膜异位症合并不孕128例临床效果分析

摘　　要：目的观察并探讨电视腹腔镜手术(LS)治疗子宫内膜异位症(EMT)合并不孕的临床效果。方法对2011年9月至2013年6月以来汉川市人民医院接诊的EMT合并不孕患者128例,采取LS的方法治疗为LS组,另外选取同期在接受开腹手术(AS)治疗的EMT合并不孕患者128例为AS组。对比两组患者手术失血量、住院天数和并发症情况,在术后随访统计患者的妊娠情况和复发情况,并据此评价LS在治疗EMT合并不孕的疗效。结果 LS组住院天数和手术失血量均显著少于AS组(P<0.01),LS组术后出现1例切口愈合不良,AS组出现7例切口脂肪液化,两组术中并发症比较差异有统计学意义(P<0.05)。LS组1年妊娠率显著高于AS组(60.9%vs 45.3%)(P<0.05)。结论LS治疗EMT合并不孕,住院天数短,出血更少,且疗效显著,是临床上较佳的治疗选择之一,应该进一步推广。Objective To observe and explore the clinical effects of laparoscopic surgery （LS） for the treatment of 128 eases with endometriosis（EMT） infertility. Methods A total of 128 patients with EMT infertility in Hanchuan City People＇s Hospital were treated by LS from Sept. 2011 to Jun. 2013 （LS group） ; another 128 patients with EMT infertility receiving abdominal surgery（AS） during the same period were selected as AS group. The intraoperative blood loss, hospital stay and complications of the two groups were compared, and pregnancy and recurrence of the situation after surgery were followed up and recorded to eval- uate the effect of LS for the treatment of EMT infertility. Results Days of hospitalization and intraoperative blood loss of LS group were significantly less than AS group （ P 〈 0.01 ） ; there was 1 case of poor incision healing after operation in LS group, and 7 cases of incision fat liquefaction in AS group, the intraoperative complication of the two groups was statistically significantly different （ P 〈 0.05 ）. Rate of pregnancy within one year of LS group was significantly higher than AS group （60.9% vs 45.3% ） （ P 〈 0.05 ）. Conclusion LS in treatment of EMT infertility, is featured with shorter hospital stay, less bleeding, and significant effect, thus is one of the preferred options in clinical and should be further promoted.
