先天性主动脉瓣畸形的超声诊断价值

摘  要:目的探讨超声心动图对主动脉瓣畸形的诊断价值。方法观察主动脉瓣的形态结构、合并畸形、主动脉瓣血流动力学变化及左心室、升主动脉的变化。结果 2叶畸形18例(94.7%),4叶畸形1例(0.53%);瓣叶不等大的患者多数合并明显的主动脉瓣狭窄(AS)和/或主动脉瓣关闭不全(AI),瓣叶等大的患者,血流动力学改变不明显;多数主动脉瓣畸形为独立疾病,少数合并其他先天心脏畸形;左心室与升主动脉的改变与主动脉瓣的血流动力学改变及患者年龄有关。结论超声心动图可以准确诊断主动脉瓣畸形及其引起的心脏改变及合并畸形。Objective To Explore the diagnostic value of echocardiogram of the aortic valve malformation. Methods Observing the morphology of deformity of the aortic valve, combined malformations, the hemodynamic changes of the aortic valve and the changes of the left ventricular and the ascending aorta.Result There are 18 cases(94.7%)of bicuspid aortic valve deformity and 1 case(0.53%)of four- leaf deformity. The most patients with not of equal size valve have apparent aortic stenosis(AS)and/or aortic insufficiency(AI)and the hemodynamic changes of the patients with equal size valve are not obvious. The most aortic valve malformation is an independent disease, and a few associated with other congenital heart malformations. The changes of the left ventricular and the ascending aorta are associated with the hemodynamic change of the aortic valve and the age of patient.Conclusion Echocardiography can accurately diagnose aortic valve malformation and its change of heare, and the combining malformations.
