超选择性插管在部分脾动脉栓塞中的临床意义

摘　　要：目的评价超选择性插管在部分脾动脉栓塞治疗中的临床意义。方法回顾性分析我院行超选择性部分脾动脉栓塞术45例,其中单纯脾功能亢进15例,原发性肝癌伴脾功能亢进22例,特发性血小板减少性紫癜5例,脾挫裂伤3例;根据脾动脉造影的表现及患者的病情,制定脾动脉栓塞的部位及范围,采用同轴导管技术进行超选择性部分脾动脉栓塞;记录术后疗效及并发症。结果所有患者均取得了满意的疗效,术后并发症少,未出现严重并发症。结论采用超选择性插管行部分脾动脉栓塞,能准确控制脾动脉栓塞位置及程度,在保证疗效的同时,明显的降低手术风险,值得临床广泛推广。Objective To evaluation of super selective intubation in clinical significance of partial splenic artery embolization.Methods Retrospective analysis of line of super selective partial splenic artery embolization 45 cases,simply spleen hyperthyroidism in 15 cases,22 cases of primary liver cancer with the spleen hyperthyroidism,idiopathic thrombocytopenic purpura in 5 cases,spleen laceration 3 cases;according to the splenic artery angiography performance and condition of the patient,the development of the site of splenic artery embolization and scope,adopts coaxial catheter technique for selective partial splenic artery embolization;record the postoperative curative effect and complications.Results All patients achieved satisfactory results,less postoperative complications,does not appear serious complications.Conclusion Line is made of super selective intubation partial splenic artery embolization,Can accurately control the position and extent of the splenic artery embolization,In ensuring the efficacy,while significantly reducing the risk of surgery,Is worth clinical popularizing widely.
