不稳定型心绞痛合并高血压患者血小板膜糖蛋白与脂蛋白(a)水平的变化及其相关性研究

摘　　要：目的观察不稳定型心绞痛(UAP)合并高血压(HBP)患者血小板膜糖蛋白(GP)与脂蛋白(a)水平的变化并分析其相关性。方法将80例UAP患者根据是否并发HBP分成HBP组(42例)与UAP组(38例),另选48例健康对照者为对照组。测定并记录所有病例的GPⅠb、GPⅡb、GPⅢa、脂蛋白(a)及各血脂水平,并对GPⅠb、GPⅡb、GPⅢa与脂蛋白(a)进行线性相关分析。结果 HBP组及UAP组的GPⅠb、GPⅡb、GPⅢa及脂蛋白(a)水平均明显高于对照组(P均<0.05),且HBP组较UAP组升高更为明显(P均<0.05)。HBP组及UAP组的血清总胆固醇、甘油三酯及低密度脂蛋白胆固醇水平均明显高于对照组(P均<0.05),但前两组之间比较差异无统计学意义(P均>0.05)。HBP组患者的GPⅠb、GPⅡb、GPⅢa分子数分别与脂蛋白(a)水平呈正相关(r=0.512、0.489、0.454,P均<0.05)。结论合并HBP的UAP患者体内GP、脂蛋白(a)水平均明显升高,并且两者密切相关。Objective To investigate the changes and correlation of platelet membrane glyeoprotein （GP） and lipoprotein （a） in unstable angina pectoris （UAP） patients with hypertension （HBP）. Methods A total of 80 UAP patients were divided into the HBP group （42 cases） and UAP group （38 cases） according to whether concurrent HBP, and 48 healthy people were served as control. Levels of GPⅠ b, GP Ⅱ b, GPⅢ a, lipoprotein （a）, and serum lipids were measured and compared in all cases. The relationships between GP Ⅰ b, GP Ⅱ b, GP Ⅲa and lipoprotein （a） were analyzed by linear correlation analysis. Results The levels of GPⅠ b, GP Ⅱ b, GPⅢ a and lipoprotein （a） were higher in the HBP and UAP groups than those in the control group, and were highest in the UAP group （all P 〈 0.05）. The levels of total cholesterol, triglyeeride and low density lipoprotein cholesterin in the HBP and UAP groups increased obviously as compared with those in the control group （all P〈 0.05）, but no significant differences were notedbetween the HBP and UAP groups （all P 〉 0.05）. Regression analysis showed that there were positive correlations between the GP Ⅰ b, GPⅡ b, GPⅢa and lipoprotein （a） in the HBP group （r = 0.512, 0.489, 0.454, all P〈 0.05）. Conclusion The levels of GP and lipoprotein （a） in UAP patients with HBP are all significantly elevated and the levels of GP are closely related with lipoprotein （a）.
