活动期类风湿关节炎患者血脂特征分析

摘　　要：目的观察活动期类风湿关节炎(RA)患者血脂特征及其临床意义。方法选取活动期RA患者(活动组)40例、非活动期RA患者(非活动组)36例与健康对照组30例,同时对受试者血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL—C)、载脂蛋白A_1(ApoA_1)、载脂蛋白B(ApoB)及C-反应蛋白(CRP)、类风湿因子(RF)、红细胞沉降率(ESR)进行检测。结果 与健康对照组比较,2组RA患者HDLC、ApoA_1显著降低(P<0.01),ESR、CRP、RF升高(P<0.01),且活动组较非活动组变化更明显(P均<0.01),而TC、TG、LDL-C、ApoB 3组间比较差异无统计学意义(P>0.05);HDL-C、ApoA_1与CRP、ESR、RF均呈显著负相关(r=-0.634、r=-0.901、r=-0.485,r=-0.625、r=-0.520、r=-0.236,P<0.05或P<0.01);TC、TG、LDL—C、ApoB与疾病活动性无明显相关(P>0.05)。结论活动期RA患者体内HDL-C、ApoA_1显著降低,这种变化与RA病情活动密切相关。Objective To observe the characteristics of serum lipids in activity rheumatoid arthritis（RA） and investigate its clinical significance.Methods Select active RA patients（active group） 40 cases,non-active RA patients（inactive group） 36 cases,and healthy control group of 30 patients,while the subjects serum total cholesterol（TC）,triglyceride（TG）,high-density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C）,apolipoprotein A,（ApoA_1）,apolipoprotein B（ApoB） and C reactive protein（CRP）,rheumatoid factor（RF）,erythrocyte sedimentation rate（ESR） were detected.Results Compared with the healthy control group,two groups of RA patients＇ HDL-C,ApoA,decreased significantly（P〈0.01）,ESR,CRP,RF increased significantly（P〈0.01）,and the active group changed more obvious than the inactivity group（P〈0.01）,and TC,TG,LDL-C,ApoB in 3 groups showed no statistically significant（P〈0.05）;HDL-C,ApoA,presented significant negative correlation with CRP,ESR,RF（r =-0.634,r =-0.901,r =-0.485,r =-0.625,r =-0.520,r =-0.236,P〈0.05 or P〈0.01）;TC,TG,LDL-C,ApoB and disease activity was not significant correlated（P〈0.05）.Conclusion In the active phase of RA,serum HDL-C and ApoA,levels decrease significantly,this change is closely related with the disease activity.
