氯沙坦与卡托普利治疗原发性高血压疗效及不良反应比较的Meta分析

摘　　要：目的系统评价氯沙坦与卡托普利治疗原发性高血压的有效性及不良反应的差异。方法计算机检索中国期刊全文数据库（1994年-2013年12月）、维普数据库（VIP）、万方数据库（1982年-2013年12月）及PUPMED（1995年-2013年12月）中关于氯沙坦、卡托普利治疗原发性高血压有效性及不良反应差异的随机对照试验（RCT）,同时对纳入的研究进行Jadad质量评分,对同质性结果采用RevMan 5.2软件进行Meta分析。结果 共纳入15篇相关文献,其中13篇有效性分析结果表明,氯沙坦与卡托普利降压有效性差异无统计学意义（OR=1.15,95%CI 0.89～1.47,P=0.28）;14项不良反应分析结果表明,氯沙坦与卡托普利的不良反应差异有统计学意义（0R=0.33,95%CI 0.24～0.45,P〈0.01）。结论氯沙坦在降压疗效上与卡托普利无明显差异,但不良反应少,较为安全。Objective To systematic review effectiveness and adverse reactions of losartan and captopril treatment difference for essential hypertension.Methods Computer retrieval Chinese Academic Journal（1994—2013 December）,VIP database（VIP）,Wanfang DATA（1982—2013 December） and PUBMED（1995—2013 December） randomized controlled trials（RCT） on losartan and captopril treatment of essential hypertension difference in efficacy and adverse reactions,while the quality of the included studies were conducted with Jadad score,the results of homogeneity using RevMan 5.2 Meta-analysis software.Results A total of 15 relevant literature were enrolled,13 effectiveness analysis showed that losartan and captopril antihypertensive effectiveness revealed no significant difference（OR = 1.15,95%CI 0.89-1.47,P = 0.28）;14 items of undesirable reaction analysis showed that losartan and captopril＇s adverse reactions difference was statistically significant（OR =0.33,95%CI 0.24-0.45,P〈0.01）.Conclusion Losartan and captopril showed no differences of effect on blood pressure,but with fewer adverse reactions and more safer.
