腹腔镜下阑尾切除术治疗妊娠合并阑尾炎有效性及安全性评估

摘　　要：目的:探讨腹腔镜下阑尾切除术在妊娠合并阑尾炎治疗中的有效性及安全性。方法:通过回顾性分析本院2007年7月-2013年1月妊娠期小于7个月合并阑尾炎并在我院采取手术治疗的病例。选取腹腔镜手术组(LA组)48例,开腹手术组(OA组)48例,通过对照分析,比较两种手术方式有效性及安全性的差异。结果:所有病例均顺利手术切除阑尾。LA组的手术时间、住院时间、下床活动时间及肛门排气时间均明显短于OA组。LA组的术后腹腔残余感染发生率及手术切口感染率也明显低于OA组。术后随访,96例患者围手术期均未发现因为手术引起早产流产等并发症。术后电话随诊,96例患者均正常分娩,胎儿出生后均未发现异常。结论:妊娠早、中期合并阑尾炎,诊断明确后,腹腔镜下阑尾切除手术安全、手术时间短、创伤少、住院时间短、手术并发症少。腹腔镜手术在围手术期不增加早产流产风险,术后随诊,未发现胎儿畸形,是一种安全、有效、微创的手术。Objective：To investigate the security and validity of laparoscopic appendectomy in treatment of appendicitis complicated with pregnancy.Methods：We retrospectively analyzed the clinical data of 96patients who suffered from the above diseases from 2007to 2013in our hospital.According to the operative method,they were divided into LA group with treatment of laparoscopic appendectomy（n=48）and OA group with treatment of open surgery（n=48）.We employed operative time,hospitalized period and the abdominal cavity infection and so on as evaluation criterions.Results： All the operation was successfully performed.Compared with OA group,LA group have less abdominal cavity and postoperative incision infection,quicker recovery,earlier postoperative exhaust and shorter hospital stay.Through telephone follow-up,there were not premature and abortion due to operations in the perioperative period as well as 96normal pregnancies.Conclusion：After diagnosing,laparoscopic appendectomy is safe,effective and mini in treatment of appendicitis complicated with pregnancy.It possesses advantages about short time of operation and hospitalization period, less complications as well as low-risk of premature and abortion.
