摘　　要：目的探讨Pilon骨折的手术治疗效果。方法对2008年5月～2013年4月我科收治的14例Pilon骨折患者进行手术治疗,按照Rtiedi-Allgower分型标准均属于Ⅲ型,采取经踝前外侧入路＋经腓骨后外侧显露胫距关节面的方法,直视下复位胫距关节面。结果骨折复位效果均较满意,其中11例创口Ⅰ期愈合,3例患者Ⅱ期愈合,骨折处均骨性愈合,无术后骨髓炎发生。结论通过内踝外侧及经腓骨后外侧入路显露的双侧入路手术方法,能完整显露胫距关节面,在直视下复位有利于移位骨折块的复位及固定,对于严重Pilon骨折具有较好的治疗效果。Objective To study the surgical treatment effects of Pilon fractures. Methods 14 patients with Pilon fractures were admitted in our department from May,2008 to April,2008 surgery treatment and all of them belonged to type Ⅲ by RtiediAllgower classification standard. The surface of tibial astragaloid joint was reestablished openly through anterolateral ankle approach and exposing tibial articular surface through post-lateral peroneal approach. Results The operation treatment of effect was satisfactory,including 11 cases with wound healing in stage Ⅰ and 3 ones in stage Ⅱ. All cases obtained synostosis and no postoperative osteomyelitis occurred. Conclusions This method can completely expose the surface of tibial astragaloid joint and is conducive to the fracture reduction and fixation,which has a good therapeutic effect on Pilon fracture.
