摘  要:目的探讨基于多b值弥散磁共振加权成像(DWI)下肝脏占位性病变信号变化的潜在临床应用价值。方法对于已知或怀疑有肝脏占位性病变的24位患者行8个b值(0,10,20,30,50,100,200,500,800 mm2/s)磁共振DWI扫描。共发现60个肝脏病变,其中肝癌11个,转移瘤22个,囊肿12个及血管瘤15个。这些病灶分成三组:囊肿组,血管瘤组及硬块组,并对他们进行定性及定量分析。结果在定性分析方面,随着b值的增加,肝囊肿组、肝血管瘤组及硬块组与周围肝实质相比DWI信号对比度分别是:逐渐降低,无明显变化及逐渐升高。在定量分析方面,随着b值的增加,感兴趣区(ROI)与周围肝实质信号比S(b)/S(0)均减小,但减小的程度不同。肝囊肿组减少程度最大、肝血管瘤组次之,硬块组减少最少。结论在无借助对比剂的情况下,多b值DWI成像技术能够用于肝脏囊肿、血管瘤及其他实质性占位的鉴别诊断。Objective To explore the value of signal changes of multi-b value in diffusion-weighted (DW) magnetic resonance (MR) imaging for tissue characterization of the focal liver masses. Methods Eight b values (0,10, 20, 30, 50, 100, 200, 500, 800 mm2/sec) of DW MR imaging were performed using 1.5-T system in 24 patients referred for evaluation of known or suspected hepatic focal masses. 60 focal liver lesions (11 hepatocellular carcinoma, 22 metastasis, 12 cysts, and 15 hemangiomas) were evaluated. All the focal masses were divided to 3 types: 1) cyst group; 2) hemangioma group; and 3) other solid masses group. Both qualitative and quantitative analysis was performed. Results In qualitative analysis, the brightness of the signal on DWI for the hepatic cysts was decreased gradually with increasing b values. The signal contrast on DWI between hemangiomas and surrounding hepatic parenchyma were not changed with increasing b value. For other solid masses of liver, with increasing b value, the brightness of the signal on DWI were increased. In the quantitative analysis, with increasing b value, the value of S(b)/S(0) in 3 groups were decreased gradually. The degree of the reduction varies, that is, the maximal in cyst group, the middle in the hemangioma group, and the minimal in the other solid group. Conclusion Multi-b value DWI could be used for differentiation of hepatic cyst, hemangioma, and other solid masses without administrating contrast.
