摘  要:目的:观察7.5%高渗盐水对重症中暑患者的临床疗效。方法:将确诊为重症中暑的病例随机分为小剂量7.5%高渗盐水治疗组(14例)和对照组(14例),观察两组患者治疗前后的血小板计数、D-二聚体、凝血功能、白细胞介素-6、昏迷时间、机械通气时间及死亡例数的变化。结果:在治疗第3天,治疗组氧合指数改善(P<0.05)。治疗后第5天,治疗组血小板计数较对照组明显上升(P<0.05)。治疗组昏迷时间、ICU住院时间较对照组明显缩短(P<0.05)。两组患者的凝血指标及并发症的发生率比较差异无统计学意义(P>0.05)。结论:7.5%高渗盐水可以改善重症中暑患者的氧合指数和血小板指标,缩短患者昏迷及ICU住院时间。Objective To evaluate clinical curative effect of 7.5% hypertonic saline in the treatment of severe heatstroke. Method A total of 28 cases of patients with severe heatstroke were randomly assigned to the treatment group (14- patients)and the control group ( 14 patients). On the basis of conventional therapy, the treatment group was given the therapy of 7. 5% hypertonic saline. The control group was only treated with conventional therapy. A contrastive analysis of the therapeutic effect between two groups patients of platelet count, D - dimer,coagulation function,interleukin 6, APACHE II score, coma time, mechanical ventilation time and death cases before and after treatment. Results On the third day of treatment, the oxygenation index was siguificandy improved ( P 〈 0. 05 ). On the fifth day, the platelet count of treatment group was significantly increased than control group( P 〈0. 05). Treatment group coma time and ICU hospital stays was siguificandy shortened(P 〈 0. 05). Two groups of patients' blood coagulation indexes and complications were no statistical differences (P 〉 0. 05). Conclusion The 7.5% hypertonic saline in the treatment of severe heatstroke can improve the patient's oxygenation index and the number of platelets, Shorten the patient's coma time and ICU hospital stays.
