摘  要:目的探讨血清25-羟维生素D(250HD)、甲状旁腺素(PTH)、血糖、血脂、尿酸、肥胖、胰岛素及生活方式等因素对血压的影响。方法资料来源于2009年贵阳市居民健康横断面调查.纳入20~79岁调查对象1403名(男性582名,女性821名)社区居民,平均(45.3±14.4)岁。所有调查对象均进行问卷调查及体格检查,用全自动生化分析仪检测空腹血糖及餐后2h血糖(2hPG)、甘油三酯(TG)、高密度脂蛋白胆固醇、血钙、血磷、血肌酐及血尿酸,用化学发光法测血清胰岛素,用放射免疫试剂盒测定250HD及门H浓度。结果通径分析结果表明,影响收缩压的因素按效应大小依次为年龄、体重指数(BMI)、胰岛素、性别、250HD、PTH、2hPG、TG,其总通径系数分别为0.453、0.294、0.120、-0.106、-0.049、0.047、0.030、0.030。影响舒张压的因素按效应依次为BMI、年龄、性别、胰岛素、2hPG、TG,其通径系数依次为0.348、0.217、-0.146、0.143、0.035、0.035。胰岛素、TG、2hPG经BMI间接作用于收缩压和舒张压,BMI、PTH、250HD直接作用于血压;年龄、性别直接和经BMI间接影响血压。结论影响成年人血压的因素以年龄和代谢紊乱指标(血清胰岛素、TG、2hPG等)最为重要,250HD和PTH可能与收缩压相关。Objective To investigate the association of serum 25-hydroxyvitamin D ( 25 OHD ), parathyroid hormone ( PTH), blood glucose, blood lipid, uric acid, obesity, insulin etc with blood pressure. Methods A total of 1 403 residents aged (45.3 ± 14.4) years were enrolled from Guiyang residents during a health survey in 2009. The standard questionnaire survey and physical examination were performed in all subjects. The fasting plasma glucose, postprandial 2 h plasma glucose (2hPG), triglyceride, high density lipoprotein-cholesterol, serum calcium, phosphorus, creatinine, and uric acid were determined by automatic biochemical analyzer, serum insulin by chemiluminescence method, and serum 25 OHD and PTH by radioimmunoassay. Results Path analysis showed that age, gender, body mass index (BMI), insulin, 25OHD, PTH, 2hPG, and triglyceride affected systolic blood pressure, with the repective path coefficients of 0. 453, 0. 294, 0. 120, -0. 106, -0. 049, 0. 047, 0. 030, and 0. 030. BMI, age, gender, insulin, 2hPG, triglyceride affected diastolic blood pressure, with the path coefficients 0.348,0. 217, -0. 146, 0. 134, 0. 035, and 0. 035 respectively. Insulin, triglyceride, 2hPG indirectly influenced blood pressure through BMI. BMI, PTH, and 250HD had a significant and direct effect on blood pressure. Gender and age directly and indirectly influenced blood pressure through BMI. Conclusion The most significant influential factors on blood pressure are age, serum insulin, triglyceride, and 2hPG in the population of Guiyang. Serum 25OHD is negatively associated with systolic blood pressure while PTH level is positively associated with systolic blood pressure.
