摘  要:目的:比较羊水泡沫试验和650 nm时的光密度(OD650)测定评估胎肺成熟度(FLM)的诊断效能,旨在为产科医生选择恰当的FLM评估方法提供依据。方法:132例患新生儿呼吸窘综合征(NRDS)单胎高危孕妇在产时直视下羊膜穿刺术抽取羊水,采用χ2检验和Fisher精确检验法比较泡沫试验和OD650测定这两种方法的产前预测NRDS的准确度。结果:132例孕妇中分娩患NRDS的新生儿10例,无论泡沫试验还是OD650测定,在足月孕妇中的准确度明显高于早产孕妇,有统计学意义(P<0.05);无论是在早产还是足月孕妇中,OD650测定的准确度明显高于泡沫试验,差异有统计学意义(P<0.05)。结论:OD650测定在产前预测NRDS的准确度明显优于泡沫试验,且较泡沫试验更客观。我们推荐临床上32W以后FLM评估优先采用OD650测定,且孕龄越大OD650测定的可靠性越高。Objective: To compare the diagnostic performances of foam test and optical density at 650 nm( OD650) for assessment of fetal lung maturity( FLM) in order to provide clinical evidence for obstetricians to choose appropriate assessment method for FLM. Methods: A total of 132 high risk single pregnant women with neonatal respiratory distress syndrome( NRDS) were selected,and amniotomy under direct vision during delivery was performed. χ2test and Fishers exact test were used to compare the accuracies of foam test and OD650for prenatal predicting NRDS. Results: Ten neonates with NRDS were born by the pregnant women,the accuracies of foam test and OD650for full- term pregnant women were statistically significantly higher than those for premature pregnant women( P〈0. 05); the accuracies of OD650was statistically significantly higher than those of foam test in full- term pregnant women and premature pregnant women( P〈0. 05).Conclusion: OD650is superior to foam test in term of prenatal predictive performance for NRDS,moreover,it is more objective. OD650is recommend for FLM evaluation after 32 gestational weeks,and the accuracy of OD650increases with gestational age.
