摘 要:目的:研究不同亚型HPV持续感染的宫颈组织中hTERC基因的表达,并讨论hTERC基因作为诊断不同级别宫颈上皮内瘤变标志物的临床意义。方法:采用荧光原位杂交(fluorescent in situ hybridization,FISH)方法,检测20例HPV转阴宫颈组织及70例持续性HPV感染宫颈组织中hTERC基因的表达;并将hTERC基因的扩增情况与组织病理学诊断结果进行比较。结果:hTERC基因在HPV转阴宫颈组织的阳性率为5.00%;在A5/A6、A7、A9及混合感染组的阳性率分别为:33.33%、53.33%、85.00%及50.00%;A9组hTERC基因扩增与对照组、A5/A6组、A7组及混合感染组比较有统计学差异(P<0.05)。hTERC基因在正常宫颈组织或炎症组、CIN1组、CIN2组及CIN3组的阳性率分别为13.79%、33.33%、66.67%及93.75%,hTERC基因的扩增与宫颈病变的程度呈正相关,hTERC基因在正常宫颈组织或炎症及CIN1组中的扩增与CIN2组、CIN3组比较有统计学差异(P<0.05)。结论:hTERC基因的扩增与宫颈感染HPV的亚型相关,hTERC基因的扩增与宫颈病变程度相关。Objective: To study the expression of human tolemerase( hTERC) gene in cervical tissue of patients with persistent infection of different subtypes of human papillomavirus( HPV),and explore the clinical significance of hTERC as a diagnostic marker of different grades of cervical intraepithelial neoplasia( CIN). Methods: FISH method was used to detect the expressions of hTERC in cervical tissue specimens of 20 cases without HPV infection( control group) and 70 cases with persistent HPV infection; hTERC gene amplification and histopathological results were compared. Results: The positive expression rate of hTERC in control group was 5. 00%; the positive expression rates of hTERC in A5 /A6 infection group,A7 infection group,A9 infection group and mixed infection group were 33. 33%,53. 33%,85. 00% and 50. 00%,respectively; there were statistically significant differences in hTERC gene amplification between A9 infection group and control group,A5 /A6 infection group,A7 infection group,mixed infection group( P〈0. 05). The positive rates of hTERC gene in normal cervical tissue or cervical inflammation group,CIN Ⅰ group,CIN Ⅱ group and CIN Ⅲ group were 13. 79%,33. 33%,66. 67%and 93. 75%,respectively; there was a positive correlation between hTERC gene amplification and the degrees of cervical lesions,there was statistically significant difference in hTERC gene amplification between normal cervical tissue or cervical inflammation group,CIN Ⅰ group and CIN Ⅱ group,CIN Ⅲ group( P〈0. 05). Conclusion: hTERC amplification is related to subtypes of HPV infection,hTERC gene amplification is correlated with the degrees of cervical lesions.
