摘 要:目的通过检测类风湿性关节炎(RA)患者血清及外周血单个核细胞中IL-37、TGF-β、IFN-γ、TNG-α和IL-1等细胞因子的水平及mRNA表达,探讨IL-37与类风湿性关节炎(RA)的相关性及作用机制。方法收集类风湿性关节炎患者及正常对照组外周血及血清,通过酶联免疫吸附试验(ELISA)及实时定量荧光(real-time)PCR的方法,检测30例RA患者(RA疾病标准评分≥6)治疗前后及20例健康对照组血清中细胞因子水平及mRNA表达,并探讨其抑制机制。通过统计学方法分析IL-37与类风湿性关节炎的相关性。结果①治疗前后RA患者组血清IL-37水平明显高于健康对照组,差别有统计学意义(P<0.05)。②RA患者血清中炎症因子IFN-γ、TNG-α、IL-1明显高于正常组,IL-37与炎症因子共同参与了类风湿关节炎的发生。③RA患者血清中IL-37与疾病严重程度呈正相关(治疗前r=0.854,P<0.05,治疗后r=0.563,P<0.05)。结论 IL-37作为一种基础的免疫抑制因子,通过其负反馈调节机制抑制炎症因子。当代偿产生的IL-37不能维持免疫平衡时,炎症因子的过度表达造成了RA患者机体损伤。通过实验证实IL-37与RA的发病及病程进展存在一定关系,为类风湿性关节炎的治疗提供新的依据。Objective To detect the IL-37,TGF-β,IFN-γ,TNG-α and IL-1 level in patients with rheumatoid arthritis( RA) and the mRNA levels in serum and peripheral blood mononuclear cells,in order to investigate IL-37 with Rheumatoid Arthritis and the correlation mechanism. Methods Collect the serum and peripheral blood mononuclear cells of RA patients and normal controls,then detect levels of various cytokines and mRNA by enzyme-linked immunosorbent assay( ELISA) and real-time PCR. Detect the serum of 30 RA patients and 20 normal controls' cytokines and mRNA level,to investigate the inhibitory mechanism. Finally,we use statistical methods to analyze the correlation between IL-37 and Rheumatoid Arthritis. Results ①The expression level of IL-37 in RA patients who before and after treatment were significantly higher than those of the normal group,the difference was statistically significant( P〈0. 05). ②The expression level of IFN-γ,TNG-α,IL-1 in RA patients were significantly higher than those of the normal controls,IL-37 and inflammatory factors involved in the development of Rheumatoid Arthritis. ③RA serum IL-37 were positively correlated with disease severity.( before treatment r = 0. 854,P〈0. 05,after treatment r = 0. 563,P〈0. 05). Conclusion IL-37 as a basis for the inhibitory factor,inhibit inflammatory cytokines by negative feedback regulation mechanism,when IL-37 can not maintain the immune balance,excessive expression of inflammatory cytokines caused the damage of RA patients. We confirmed experimentally,there was a certain relationship between IL-37 and the onset and progression of RA,which provided a new basis for treatment of Rheumatoid arthritis.
