抗纤溶药物在脊柱手术中应用的有效性和安全性Meta分析

摘　　要：目的评价抗纤溶药物在脊柱手术中的有效性和安全性。方法计算机检索Medline(1966年—2013年11月)、PubMed(1966年—2013年11月)、EMBase(1966年—2013年11月)、Cochrane图书馆(2013年第3期)、中国生物医学光盘数据库(1978年—2013年11月)、中国生物医学文献数据库(1978年—2013年11月)和维普中文科技期刊数据库(1978年—2013年11月)中关于抗纤溶药物在脊柱手术中应用的随机对照试验,查阅所有检出文献和相关综述的参考文献作为补充资料,均截止到2013年11月。采用RevMan 5.1软件进行统计分析。结果共纳入9篇高质量的随机对照试验,包含664例受试者。Meta分析结果显示,与对照组相比,抗纤溶药物能够减少失血量〔均数差=-288.8,95%CI(-467.49,-110.19),P=0.002〕,降低输血率〔OR=0.57,95%CI(0.37,0.87),P=0.009〕和输血量〔均数差=-242.76,95%CI(-422.57,-62.95),P=0.008〕,而深静脉血栓的发生率间差异无统计学意义(P>0.05)。结论抗纤溶药物可有效减少脊柱手术失血,降低输血率及输血量,且不增加深静脉血栓的危险。Objective To assess the efficacy and safety of antifibrinolytic agents in spine surgery.Methods Randomized controlled studies of antifibrinolytic agents in spine surgery were searched for in Medline（1966—2013.11）,PubMed（1966—2013.11）,EMBase（1966—2013.11）,Cochrane（2013,Section 3）,Chinese Biomedical Literature Database（1978—2013.11）,VIP Chinese Scientific Journals Database（1978—2013.11）,and references of related review articles selected for supplementary information.RevMan 5.1 software was used to carry out statistical analysis.Results A total of 9 high-quality randomized controlled studies including 664 subjects.By meta analysis,as compared with control group,antifibrinolytic agents can reduce blood loss 〔mean difference =-288.8,95% CI（-467.49,-110.19）,P = 0.002 〕,decrease blood transfusion rate 〔odds ratio = 0.57,95% CI（0.37,0.87）,P = 0.009〕and transfusion volume 〔mean difference =-242.76,95% CI（-422.57,-62.95）,P = 0.008 〕,but there was no significant difference in incidence of deep vein thrombosis（P〈0.05）.Conclusion Antifibrinolytic agents can effectively reduce blood loss,blood transfusion rate and volume without increasing the risks of deep vein thrombosis.
