酸枣仁汤治疗慢性乙型肝炎睡眠障碍临床研究

摘　　要：目的:观察酸枣仁汤治疗肝血虚型慢性乙型肝炎睡眠障碍的临床疗效,并探讨其在ABO血型中的分布规律。方法:将130例患者按随机数字表法分别对照组65例和治疗组65例,治疗组采用酸枣仁汤治疗,对照组采用艾司唑仑治疗。将130例患者按睡眠障碍特征分为难入睡、多梦、早醒、睡眠间断、彻夜不寐五种类型,观察各睡眠类型在ABO血型的分布情况。观察两组患者治疗前后PSQI积分、SAS积分、SDS积分的改变情况及不良反应、反跳性。结果:各睡眠障碍类型患者在血型构成的差异存在统计学意义,O型血主要分布在多梦型;A型血主要分布在多梦型和难入睡型;B型血主要分布在难入睡型;AB型血主要分布在彻夜不寐型。经1个疗程治疗后治疗组有效率为73.83%,对照组有效率为61.54%,具体体现在睡眠潜伏期、睡眠总时间、睡眠效率等方面。治疗组不良反应(7.69%)及反跳性失眠(3.08%)发生率均明显低于对照组(不良反应发生率35.38%、反跳性失眠发生率12.31%)。结论:肝血虚型慢性乙型肝炎伴睡眠障碍患者的ABO血型分布在各睡眠障碍类型间存在差异。酸枣仁汤改善肝血虚型慢性乙型肝炎睡眠障碍疗效显著具有不良反应少、反跳性低的特点。Objective ： Discuss the correlation of Liver blood deficiency type chronic hepatitis B sleep disturbance type and the ABO blood group system and observe the effects of Suanzaoren Decoction in the treatment of it. Methods ：130 cases of patients according to their sleep disturbance characteristics into hard to sleep, dreams, waking early, sleep all night insomnia intermittent, five types of sleep, observe various types and ABO correlation between ABO blood groups. The patients were randomly divided into two groups, respectively, to the Suanzaoren Decoction（ treatment group） and Ai Sle（ control group） were treated with. Observation of the two groups, PSQI, SAS, SDS change before and after treatment. Observation of adverse reactions and the rebound condition. Results ：The patients with sleep disorders in type of blood type composition differences of statistical significance, O type blood are mainly distributed in the dreamy type; type A blood are mainly distributed in the dreams and difficult to get to sleep;blood type B mainly distributed in hard to sleep type;type AB blood type is mainly distributed in the night of insomnia. After a course of treatment （4 weeks） after treatment, the total effective rate of treatment group 73.83%, control group in the total efficiency of 61.54%, embodied in the sleep latency,total sleep time, sleep efficiency etc. Treatment of patients with adverse reactions （7.69%） and rebound insomnia（ 3.08% ） incidence were significantly lower than those in control group （35.38% , 12.31% ）. Conclusion：The liver of chronic hepatitis B with sleep disorders in patients with ABO blood group distribution in various types of differences between sleep disorders. Suanzaoren Decoction improving liver blood deficiency type chronic hepatitis B sleep disorders efficacy with fewer adverse reactions, bounce low characteristics.
