PVP与PKP并发症原因分析及预防

摘　　要：目的分析经皮椎体成形术(percutaneous vertebroplasty,PVP)和经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗骨质疏松性椎体压缩骨折术中术后并发症情况及预防策略。方法回顾性分析2008年1月至2013年1月在我院C型臂机下进行PVP及PKP的238个病例共374个椎体,发生并发症45例共67个椎体,对发生并发症的45例进行分析。结果总共有45例患者67个椎体出现了术中术后并发症,发生率为18.9%。其中37例发生骨水泥渗漏,2例损伤神经根,1例穿刺部位发生血肿,3例术中出现恶心、呕吐并低血压状态,3例随访中出现病椎或邻椎再骨折。结论 PVP及PKP发生一般并发症并不少见,严重并发症较少,预防并发症的关键是严格掌握手术适应证和禁忌证,标准规范熟练的手术操作,术中C型臂严密监测能减少并发症的发生率。Objective To nalysis of postoperative complications and prevention strategies in the percutaneous vertebroplasty（ PVP） and percutaneous kyphoplasty（ PKP） in osteoporotic vertebral compression fractures. Methods A retrospective analysis of 238 cases（ 374 vertebrae） who underwent PVP or PKP under C-arm X-ray in our hospital from January 2008 to January 2013. Complications occurred in 45 cases（ 67 vertebrae）. The 45 cases were analyzed. Results A total of 45 cases（ 67 vertebrae） had postoperative complication,the incidence was 18.9%. Of which 37 cases occurred bone cement leakage,1 case occurred nerve root occurred,1 case occurred puncture site hematoma,3 cases appeared fracture again in injured vertebral or adjacent vertebral during follow-up again. Conclusion Generally complications of PVP and PKP are not seldom. Serious complications seldom occurred. The key to prevent complications is strictly master the indications and contraindications. Standard skilled operative,C-arm closely monitoring can reduce the incidence of complications during operation.
