摘  要:目的采用大鼠右足底注射完全弗氏佐剂(complete freund's adjuvant,CFA)建立大鼠慢性炎性痛模型,观察牛肾上腺髓质22肽(bovine adrenal medulla 22,BAM22)对CFA致大鼠慢性炎性痛和电针(electroacupuncture,EA)镇痛作用的影响。方法将42只脊髓鞘内插管成功的健康雄性SD大鼠右足底注射CFA建立大鼠慢性炎性痛模型,造模后将大鼠随机分为生理盐水组、BAM22组、BAM22+纳洛酮组、生理盐水+EA组、BAM22抗体组和BAM22抗体+EA组,每组7只。生理盐水+EA组和BAM22抗体+EA组均在造模24 h后开始介入电针治疗,电针刺激大鼠双侧足三里、昆仑穴,刺激参数为疏密波,频率2/100 Hz,刺激强度1 mA,时间30 min,每日1次,连续7 d;余组仅以相应固定,不予电针处理。造模后第7天,各组大鼠鞘内注射相应试剂。分别检测造模前和造模后24 h、6 d、7 d时大鼠右足的热缩腿潜伏期(thermal paw withdrawal latency,T-PWL)作为痛阈指标。结果 BAM22多肽、电针组均可明显延长大鼠患侧T-PWL,鞘内注射纳洛酮能部分阻断BAM22多肽对大鼠痛阈的影响;BAM22抗体也可明显反转电针的T-PWL延长效应。结论 BAM22多肽和电针均能显著抑制CFA大鼠慢性炎性痛,电针镇痛作用可能存在与BAM22多肽相关的非阿片机制。Objective To investigate the effect of bovine adrenal medulla 22 peptide (BAM22) on CFA-induced chronic inflammatory pain and electroacupuncture (EA) analgesia in a rat model of inflammatory pain made by right sole injection of complete Freund's adjuvant (CFA). Methods A rat model of inflammatory pain was made by injection of CFA into the right soles of 42 healthy male SD rats with successful intrathecal catheterization. After model making, the rats were randomly allocated to normal saline, BAM22, BAM22+naloxone, normal saline+EA, BAM22 antibody and BAM2 antibody +EA groups, seven rats each. The normal saline+EA and BAM2 antibody +EA groups were treated with electroacupuncture at 24 hrs after model making. Rat bilateral points Zusanli and Kunlun were provided with sparse-dense wave electroacupuncture stimulation at a frequency of 2/100 Hz and an intensity of 1 mA, 30 min once daily, for seven consecutive days. The other groups were given only the same fixation and no treatment. Every group was injected with the corresponding reagents at seven days after model making. Thermal paw withdrawal latency (T-PWL) as a pain threshold index was measured before model making and at 24 hrs and six and seven days after. Results Affected-side T-PWL lengthened markedly in both BAM22 peptide and electroacupuncture groups of rats. lntrathecal injection of naloxone partly counteracted the effect of BAM22 peptide on pain threshold in the rats. BAM22 antibody also obviously reversed the prolonging effect of electroacupuncture on T-PWL. Conclusions Both BAM22 peptide and electroacupuncture can markedly alleviate rat CFA-induced chronic inflammatory pain. The analgesic effect of electroacupuncture may involve a BAM22 peptide-related non-opiate mechanism.
