2型糖尿病患者多层螺旋CT胰腺灌注成像研究  被引量:3

摘  要:目的探讨多层螺旋cT胰腺灌注成像在反映2型糖尿病患者胰腺的微循环方面应用价值。方法选取2012年1月~2013年12月在华北石油管理局总医院确诊并治疗的40例2型糖尿病患者为研究组,依据患者的病程时间和血糖控制情况进行分为4个组(病程时间≥10年组、病程时间〈10组、血糖控制理想组、血糖控制不佳组),并选取同期因其他原因在华北石油管理局总医院行上腹部cT增强检查的20例患者为对照组,分别测量胰腺头、颈、体、尾部的血流量(BF),血流容积(BV),平均通过时间(MTT),达峰时间(TTP),表面通透性(Ps)的平均值,进行统计学分析。结果糖尿患者与正常人比较,胰腺头、颈、体尾的BV、BF均减低,MTT和TTP延长,Ps增高,病程≥10年组胰腺头、颈、体尾的BV、BF均低于病程〈10年组,胰腺头、颈部MTT延长,TTP延长,Ps增高。血糖控制不佳组也同样均低于血糖控制理想组。结论应用多层螺旋cT胰腺灌注成像能够反映2型糖尿病患者胰腺的微循环状态,有一定临床价值。Objective To investigate the value ofCT perfusion imaging in pancreatic microcirculation in patients with type 2 diabetes. Methods 40 cases with type 2 diabetes treated in our hospital from January 2012 to December 2013 were selected for the study group, and then based on the time and duration of the patient's blood glucose control situation 40 cases were divided into four subgroups (duration ≥10 years group, duration of time 〈10 group, ideal glycemic control group, poor glycemic control group). 20 subjects accepted CT enhancement of upper abdomen for Other reason in corresponding period were selected as the control group.The average value of BF. BV, MTT, TTP and PS was measured respectively in head. neck. body and tail of pancreas and statistically was analyzed. Results The value in the study group of BF. BV was reduced and MTT. TTP was lengthened and PS was elevated in head. neck. body and tail of pancreas compared to the control group.So was duration 〉 10 years group compared to duration of time 〈10 group, and poor glycemic control group compared to ideal glycemic control group. Conclusion The clinical application of pancreatic CT perfusion conduce to evaluation the microcirculation change of pancreas in patients with type 2 diabetes.
