摘 要:目的探讨血清尿酸(SUA)水平与慢性肾脏疾病(CKD)患者心室肥厚的相关性,尤其是这种相关性是否存在性别差异。方法连续性入选2010年4月~2013年10月就诊于我院的216例CKD患者。使用超声心动图评估左室质量指数及左室肥厚(LVH)情况。结果左室肥厚的发生率为男性58%,女性47%。在多因素回归分析中,男性的SUA水平与左室质量指数无关,而女性的SUA水平与左室质量指数明显相关(β=0.27,P=0.02)。同时,对于男性而言,糖尿病(OR=4.41,P=0.01)与左室肥厚相关,而对于女性患者,年龄(OR=1.13,P〈0.01)、高血压(OR=7.38,P=0.03)和SUA(OR=1.91,P=0.03)均与左室肥厚相关。结论女性CKD患者SUA水平与左室质量指数和左室肥厚相关,而男性患者未发现这种相关性。女性患者SUA水平与左室肥厚之间的相关性比男性更明显。Objective To determine whether SUA being associated with cardiac hypertrophy in CKD patients,focusing on sex differences. Methods 216 CKD patients were enrolled in this study. Left ventricular mass index(LVMI) and left ventricular hypertrophy(LVH) were assessed by echocardiography. Results The prevalence of LVH was 58% in male and 47% in female. In multivariate linear regression analysis,SUA levels did not correlate with LVMI in male,whereas SUA was independently associated with LVMI in female(β=0.27,P=0.02). Multivariate logistic regression analysis also revealed that diabetes mellitus(OR=4.41,P=0.01) was associated with LVH in male,whereas age(OR=1.13,P=0.01),hypertension(OR=7.38, P=0.03) and SUA(OR=1.91,P=0.03) were associated with LVH in female. Conclusion In female CKD patients,SUA levels were associated with LVMI and LVH,whereas there was no association in male patients. These observations suggest that an association between SUA levels and the development of cardiac hypertrophy is more likely in female than in male.
