摘  要:目的:探讨婴幼儿人巨细胞病毒(human cytomegalovirus, CMV)感染后体内病毒载量不同与脑干听觉诱发电位(brainstem auditory evoked potential , BAEP)异常之间的关系。方法经病原学诊断为CMV感染的住院患儿160例,年龄10天~3个月,尿CMV-DNA拷贝数为1.797×10^4/ml ~5.62×10^7/ml ,按照尿病毒拷贝数的幂指数分为104(n=72)、105(n=40)、106(n=28)和107(n=20)4组,分别对其进行BAEP检测。结果尿CMV-DNA拷贝数的不同与BAEP的异常改变之间存在关联。①客观听阈值随CMV-DNA拷贝数的增加而增高( P<0.01)。②Ⅰ、Ⅲ、Ⅴ波的潜伏期随CMV-DNA拷贝数的增加而延长(P<0.01),Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波间期随CMV-DNA拷贝数的增加而延长(P<0.01)。结论婴幼儿CMV感染后尿液中病毒载量增高与进行性感音神经性听力丧失发生风险增高有关。Objective To investigate the relationship between virus load in urine and abnormal brainstem auditory evoked potential ( BAEP) in human cytomegalovirus ( CMV) infected infants .Methods 160 infants aged 10 days to 3 months diagnosed etiologically as CMV infection were included in this study .The copy number of CMV -DNA in urine ranged from 1.797 ×10^4/ml to 5.62 ×107/ml.The infants were divided into 4 groups according to the power and expo-nent of CMV-DNA in urine:104(n=72),105(n=40), 106(n=28), and 107(n=20).BAEP was detected for each group.Results There was a positive-relationship between the abnormal BAEP and the copy number of CMV -DNA in urine.The increased objective auditory threshold , prolonged incubation period of wave Ⅰ,Ⅲ,Ⅴand inter-phase be-tween the wave of Ⅰ-Ⅲ, Ⅲ-Ⅴ, Ⅰ-Ⅴ were correlated with increased copy number of CMV -DNA in urine in CMV infected infants (P<0.01).Conclusion In infants with CMV infection, progressive sensorineural hearing loss is associated with increased urine CMV load .
