祛湿清结汤联合清热祛毒方灌肠治疗溃疡性结肠炎随机平行对照研究

摘　　要：[目的]观察祛湿清结汤联合清热祛毒方灌肠治疗溃疡性结肠炎疗效。[方法]使用随机平行对照方法,将64例门诊患者按掷骰子法分为两组。对照组32例柳氮磺胺嘧啶片(SASP),活动期1.0g/次,4次/d;缓解期0.5g/次,3次/d,口服。治疗组32例①祛湿清结汤(苍术15g,陈皮5g,茯苓、薏苡仁各20g,砂仁10g,木香7g,佛手、槐花、地榆各10g,鸡内金15g,麦芽20g),1剂/d,水煎200mL,早晚口服;大便脓血较多加败酱草、秦皮;便血较多加仙鹤草、紫草;腹痛较甚加徐长卿、延胡索;伴发热加金银花、葛根;久泻不止赤石脂、诃子;畏寒怕冷加干姜、附子。②清热祛毒方(黄连5g,茯苓15g,木香、槐花、地榆各10g,两面针15g,白及30g,苍术、赤芍各15g,丹皮10g),1次/晚,水煎100mL,保留灌肠。连续治疗15d为1疗程。观测临床症状、不良反应。连续治疗2疗程,判定疗效。随访60d,观测复发率。[结果]治疗组痊愈15例,有效13例,无效4例,总有效率87.50%。对照组痊愈10例,有效14例,无效8例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。[结论]祛湿清结汤联合清热祛毒方灌肠治疗溃疡性结肠炎效果显著,值得推广。[Objective] To observe the wet cleaning decoction combined with heat Qudu Decoction enema treatment of ulcerative colitis. [Methods] Using randomized controlled methods,64 patients were in the dice were divided into two groups. A control group of 32 cases of sulfasalazine Sulfadiazine Tablets（SASP）,active 1.0g/, 4 times /d;remission of 0.5g/ times,3 /d,oral. The treatment group of 32 cases of Qushi clean soup（Cangshu 15g,Chenpi 5g,Fuling,Yiyiren each 20g,Sharen 10g,muxiang 7g,Foshou,Huaihua,Diyu each 10g, Jineijin 15g,maiya 20g）,1 doses of /d,water simmer in water 200mL,sooner or later oral;bloody stool more plus Baijiangcao,Qingpi;and the blood more add Xianhecao,Zicao;abdominal pain is very add Xuchangqing, Huyansauo,Corydalis yanhusuo;fever plus Jinyinhua,Gegen;more than jiuxie halloysite add Chishizhi,Kezi; cold chills,add Ganjiang,Fuzi. The clearing heat and eliminating toxicity（Huanglian 5g,Fuling 5g,Muxiang, Huaihua,Diyu each 10g,Liangmianzhen 15g,Baiji 30g,Cangshu,Chishao each 15g,Danpi 10g）,1 / night, Decoction 100mL,retention enema. Continuous treatment with 15d for 1 courses. Observation of clinical symptoms,adverse reaction. Continuous 2 courses of treatment,the curative effect. Follow up 60d,observing the recurrence rate. [Results] The treatment group cured 15 cases,effective 13 cases,invalid 4 cases,the total efficiency of 87.50%. The control group cured 10 cases,effective 14 cases,invalid 8 cases,the total efficiency of 75%. The treatment group was better than control group（P〈0. 05）. [Conclusion] Qushi clear soup combined heat Qudu Decoction enema in treatment of ulcerative colitis effect is significant,worthy of promotion.
