循证护理预防肿瘤患者PICC术后机械性静脉炎的效果

摘　　要：目的 探讨循证护理在肿瘤患者中预防PICC导管相关性静脉炎的临床效果.方法 将123例采用PICC导管行化疗的肿瘤患者随机分为观察组和对照组,对照组仅行常规护理,观察组给予循证护理,观察并比较两组患者置管后静脉炎的发生率、发生时间及平均置管时间.结果 观察组患者导管相关性静脉炎的发生率为6.2％,明显低于对照组的20.7％,且差异有统计学意义（x2 =5.23,P＜0.05）.而且,观察组患者静脉炎发生的时间（7.1±2.1）d明显晚于对照组（3.3±1.4）d,平均置管时间（103.6±10.2）d明显长于对照组（72.3±11.5）d,差异均有统计学意义（t分别为3.26,4.65;P＜0.05）.结论 对行PICC置管的肿瘤患者进行循证护理能显著降低导管相关性静脉炎的发生率,延长置管时间,值得临床推广和进一步深入研究.Objective To explore the effects of evidence-based nursing on preventing mechanical phlebitis in cancer patients with peripherally insert central catheter.Methods Totals of 123 cancer patients with peripherally insert central catheter in sixth people＇ s hospital of Shaoxing city were randomly divided into experiment group and control group.And 58 patients in the control group received only traditional nursing while the evidence-based nursing was carried on the 65 patients in experiment group.Then,the occurrence time of mechanical phlebitis,the incidence of mechanical phlebitis and the mean catheterization time were compared between the two groups.Results The incidence of mechanical phlebitis in experiment group was 6.2%,significantly lower than that of control group which was 20.7% （x2 =5.23,P 〈 0.05）.The occurrence time of mechanical phlebitis was later than that of the control group [（7.1 ± 2.1） vs （3.3 ± 1.4） d],and the mean catheterization time was significantly longer than that of the control group [（103.6 ± 10.2） vs （72.3 ± 11.5） d],and the differences were statistically significant （t =3.26,4.65,respectively ;P 〈 0.05）.Conclusions Evidencebased nursing in cancer patients with peripherally insert central catheter can significantly reduce the incidence of mechanical phlebitis,so it is worthy of clinical practice.
