摘 要:目的探讨尼古丁对大鼠骨折愈合及血管内皮生长因子表达的影响。方法 60只雄性SD大鼠,随机分为模型组、轻度尼古丁组、重度尼古丁组。制造骨折前后尼古丁生物效应,无菌状态下手术,在大鼠桡骨中下1/3交界处造横断骨折愈合模型;于术后3、7、14、21d每组随机处死5只大鼠,取左侧桡骨标本作为观察对象,使用HE染色法检测骨痂厚度及成熟度和免疫组化法检测骨痂中VEGF表达水平三个指标。结果造模后第3天,各给药组与模型组比较,标本骨痂厚度及VEGF阳性细胞数差异无统计学意义(P>0.05),显微镜下骨痂成熟度无差异;造模后第7天、第14天和第21天,各给药组与模型组比较,标本骨痂厚度及VEGF阳性细胞数差异有统计学意义(P<0.05,P<0.01),给药组的VEGF表达高于模型组,但是给药组骨痂厚度低及骨痂成熟度较模型组低;轻度尼古丁组、重度尼古丁组VEGF阳性细胞数组间差异有统计学意义(P<0.05),轻度尼古丁组VEGF表达均高于重度尼古丁组。结论尼古丁影响骨痂增殖分化使骨痂的形成减少,抑制骨的成熟改造,而延缓骨折的愈合过程;另一方面尼古丁可促进血管内皮VEGF表达,而可能促进内皮细胞增生从而促进血管新生,而且不同水平的尼古丁对VEGF表达影响不同。Objective To explore the effects of nicotine on rat fracture healing and vascular endothelial growth factor (VEGF) expression. Methods 60 male Sprague-Dawley rats were randomly divided into model group, mild nicotine group and severe nicotine group. Manufacture of biological effects of nicotine; surgery under sterile conditions, causing a transverse fracture healing model in the junction of the lower 1/3 of the rat radial'; 3,7,14,21 days after surgery,5 rats each group were sacrificed randomly ,take the left radial specimens as the observed object, using HE staining detection the callus thickness and maturity, immunohistochemical method detection callus VEGF expression level. Results 3 days after modeling, the difference in specimens VEGF positive cells and callus thickness between each treatment group and the model group was not statistically significant ( P 〉 0. 05 ), no difference in the maturity of the callus under the microscope ;7,14 days and 21 days after modeling, the difference in specimens VEGF positive cells and callus thickness between each treatment group and the model group was statistically significant (P 〈 0. 05, P 〈 0. 01 ) , in the expression of VEGF, treatment group was higher than model group, but the treatment group callus thickness and maturity was low compared with the model group;the difference in specimens VEGF positive cells between the the mild nicotine group and severe nicotine group was also statistically significant (P 〈 0. 05 ). Mildnicotine group were higher than the expression of VEGF in the severe nicotine group. Conclusions Nicotine affects the proliferation and differentiation of callus reduced callus formation, inhibition of bone maturity transformation, and delays the healing process of fracture ; On the other hand, nicotine may promote vascular endothelial VEGF expression, which may promote the proliferation of endothelial cells to promote angiogenesis, the different levels of nicotine on VEGF expression impact are also different.
