精索静脉曲张不育患者睾丸组织弹性成像表现及精液质量相关性分析

摘　　要：目的利用超声弹性成像组织弥散定量分析技术(UE)探讨双侧精索静脉曲张(VC)引起男性不育患者睾丸组织硬度改变与精液质量之间的关系。方法将57例双侧精索静脉曲张患者(精索静脉曲张组)分为Ⅰ级(20例)、Ⅱ级(29例)、Ⅲ级(8例)3个亚组,另设正常对照组30例。对所有受检者进行常规超声扫查后,使用实时组织超声弹性成像技术进行扫查,并行组织弥散定量分析,测量感兴趣区域(ROI)内应变均值。精液则做常规检查。结果与对照组比较,病例组睾丸组织应变均值升高(P<0.05),而精子密度则降低(P<0.01)。VC组内比较,随着VC级别的递增,双侧睾丸组织应变均值逐渐增高,而精子密度则渐降。相关分析显示,VC组睾丸组织应变均值与精子密度呈负相关(r=-0.773,P<0.05)。结论睾丸组织应变均值可作为评价VC患者睾丸组织硬度并预测VC不育患者精液质量的参考指标。Objective To observe the relation between the organization hardness of testicular tissue changes detected by the ultrasonic elastic quantitative analysis of organization diffusion technique and the semen quality of male infertile patients caused by bilateral varicocele（VC）.Methods 57 patients with bilateral varicocele（57 VC group） were divided into VC I subgroup （n =20）,VC Ⅱ subgroup （n =29） and VC Ⅲ subgroup （n =8）,and 30healthy subjects were enrolled as control group.All subjects underwent conventional ultrasound and then ultrasound real-time tissue elastography.The quantitative analysis of organization diffusion was performed,and the average strain of ROI was measured,and the routine examinations of semina were performed.Results Compared with the control group,the hardness of testicular tissue was higher（P 〈0.05）,and the sperm density was lower（P 〈0.01）,the hardness of testicular tissue showed increasing tendeny along the increase of VC severity,but the sperm density became decreasing.The linear correlation nanlysis showed that the hardness of testicular tissue of VC group were respectively negatively correlated with the sperm denity（r =-0.773,P 〈0.05）.Conclusions The left hardness of testicular tissue detected by the ultrasonic elastic quantitative analysis of organization diffusion technique may be a reference index for forecasting the semen quality of left VC patients.
