定量组织速度成像评价酒精性心肌病患者左心房功能

摘　　要：目的应用定量组织速度成像技术评价酒精性心肌病患者左心房功能。方法选取酒精性心肌病男性患者30例作为试验组,选取同期正常成年男性30例设为对照组,Simpson法获取两组常规参数:左心房最大容积(LAVmax)、左心房p容积(LAVp)、左心房最小容积(LAVmin)、左心房最大直径(LAMD),并计算左心房主动射血分数(LAAEF)。应用定量组织速度成像技术测量左心房间隔、侧壁的心缩期组织运动速度S sia、S lat,心室舒张早期组织运动速度E sia、E lat,心室舒张晚期组织运动速度A sia、A lat。比较两组各参数并衡量心室舒张晚期房间隔、左心房侧壁组织运动速度与LAAEF的相关性。结果与正常对照组相比,LAVmax、LAVp、LAVmin、LAMD及LAAEF增大,差异均具有统计学意义(P均<0.01)。与正常对照组相比,左心房组织运动速度S sia、S lat、E sia、E lat减低,A sia、A lat增高,差异均具有统计学意义(P均<0.01)。左心房间隔组织运动速度A sia与LAAEF相关性显著(r=0.52,P<0.01),左心房侧壁组织运动速度A lat与LAAEF相关性显著(r=0.59,P<0.01)。结论定量组织速度成像能够方便准确评估酒精性心肌病左心房功能变化情况。Objective To study atrial function changes in patients with alcoholic .cardiomyopathy （ ACM） by quantitative tissue velocity imaging .Methods This study investigated 30 patients with ACM and 30 age-matched healthy control subjects .Left atrial maximum volume （ LAVmax） ,left atrial pre-atrial contraction volume （ LAVp） ,left atrial minimum volume （ LAVmin ） , left atrial maximum diameter （ LAMD ） and left atrial active ejection fraction （ LAAEF） were measured using single-plane simpson method .Peak myocardial velocity during early diastole waves （Esia,Elat）,ventricle contraction waves（Ssia,Slat）and late diastole waves（Asia,Alat）were recorded in the atrial septum and left atrial lateral wall .Correlation analysis was conducted between the left atrial septum and lateral wall systolic velocities and LAAEF .Results LAVmax, LAVmin, LAVp, LAMD, LAAEF significantly increased （ P ＆lt;0.01）.Esia,Elat,Ssia,Slat,significantly decreased （P＆lt;0.01）,whereas.Asia,Alat significantly increased （P＆lt;0.01）.In addition,Asia and Alat were closely correlated with LAAEF （rsia =0.52,rlat =0.59,both P＆lt;0.01）.Conclusion QTVI could be used to evaluate the left atrial function accurately in patients with ACM .
