芬兰糖尿病风险评分和社区人群代谢综合征发病风险的相关性分析

摘　　要：目的分析芬兰糖尿病风险评分（FINDRISC）和代谢综合征的相关性。方法以来自江苏徐州地区的1780名社区人群为研究对象,检测受试者甘油三酯（TG）、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、空腹血糖（FBG）和餐后2 h血糖（PBG）等相关生化指标。FINDRISC问卷由专业营养师询问评估,包括年龄、体重指数、腰围、体力活动、饮食习惯、有否高血压或服降压药史、有否血糖升高史和家族糖尿病史。结果用FINDRISC来评估代谢综合征的受试者工作曲线下面积为0.78（95%CI 0.76∽0.80）,其中男性为0.76（95%CI 0.74∽0.79）,女性为0.80（95%CI 0.75∽0.85）。而在最佳诊断截点（FINDRISC=12.5分）,诊断代谢综合征的敏感度为70.7%,特异度为74.7%。FINDRISC与代谢综合发生的风险相关,校正相关参数（年龄、腰围、体重指数、空腹血糖）后,这种相关性依然存在。结论 FINDRISC和代谢综合征密切相关,是一种较为实用的评估代谢综合征的指标。Objective To investigate the relationship between the Finnish Diabetes Risk Score（FINDRISC） and Metabolic Syndrome. Methods The study samples were from a community-based health examination survey in Xuzhou, Jiangsu province of China. A total of 1780 subjects were included in the present study. Serum triglyceride（TG）, total cholesterol（TC）, high density lipoprotein cholesterol（HDL-C）, low density lipoprotein cholesterol（LDL-C）and glucose were measured. The FINDRISC questionnaire was applied by registered nutritionists. Points were attributed for the following parameters： age, self-reported use of blood pressure medication, history of high blood glucose, physical activity at least 4 h a week, daily consumption of vegetables, fi＇uits, and berries, and measured body mass index （BMI, kg/m2） and waist circumference, and family history of diabetes. Results The area under the receiver operating characteristics curve for steatosis was 0.78 （95% CI 0.76-0.80）; 0.76 in men （95% CI 0.74-0.79） and 0.80 （95% CI 0.75-0.85） in women. The optimal cutoff value for detecting the Metabolic Syndrome was a FINDRISC greater or equal to 12.5, with sensitivity of 70.7% and specificity of 74.7%. FINDRISC was associated with the risk of Metabolic Syndrome, the associations is still there, after adjust for the following parameters（age, waist circumference, body mass index, fasting blood sugar）. Conclusions The FINDRISC was found to be correlated with metabolic syndrome. The FINDRISC is a useful clinical practical evaluation index of metabolic syndrome.
