摘  要:目的探讨血清25羟基维生素D(25-OH-D)与急性冠状动脉综合征(ACS)的关系。方法入选符合国际心脏病的诊断和命名标准患者152例,分为正常对照组36例,稳定型心绞痛组35例,ACS组81例,其中ACS组分为不稳定型心绞痛组40例,急性心肌梗死组41例。采用酶联免疫法对入选对象进行血清25-OH-D、基质金属蛋白酶9(MMP-9)水平的测定。结果 ACS组血清25-OH-D水平显著低于SAP组和正常对照组[(225.78±38.48)ng/Lvs.(325.00±42.74)ng/L,(339.69±35.30)ng/L,P<0.01];急性心肌梗死组血清25-OH-D水平低于不稳定型心绞痛组[(219.75±31.53)ng/Lvs.(231.97±44.04)ng/L,P<0.01],正常对照组与稳定型心绞痛组血清25-OH-D水平无统计学差异(P>0.05)。ACS组血清MMP-9水平显著高于稳定型心绞痛组和正常对照组[(563.36±48.18)ng/Lvs.(341.39±46.23)ng/L,(330.03±48.06)ng/L,P<0.01];急性心肌梗死组血清MMP-9水平高于不稳定型心绞痛组[(598.88±29.34)ng/Lvs.(526.95±34.56)ng/L,P<0.01],正常对照组与稳定型心绞痛组血清MMP-9水平无显著统计学差异(P>0.05)。结论血清25-OH-D在ACS发生发展过程中呈降低趋势,其变化可能与参与斑块的炎症反应有关,在ACS发病中起一定作用。Objective To investigate the relationship of the serum 25-hydroxy vitamin D with acute coronary syndrome(ACS). Methods 152 patients who conformed to standard were chosen. The patients were divided into ACS group( n = 81 ), SAP group( n = 36 )and NC group (n = 35 ). By clinical conditions, we also divided ACS group into AM1 group( n = 41 )and UAP group (n = 40).The level of 25-hydroxy Vitamin D, MMP-9 were measured by ELISA in every group. Results The serum level of 25-hydroxy vitamin D was significantly lower in ACS group than in SAP group, control group [ ( 225.78 ± 38.48 ) ng/L vs. ( 325.00 ± 42. 74 ) ng/L, ( 339. 69 ± 35. 30 ) ng/L, P 〈 0. 01 ]. The serum level of 25-hydroxy vitamin D was lower in AMI group than in UAP group [ (219. 75 ± 31.53 )ng/ L vs. (231.97 ± 44. 04 )ng/L,P 〈 0.01 ]. No significantly statistics significance was found between SAP group and control group(P 〉 0. 05 ). The serum level of MMP-9 was significantly higher in ACS group than in SAP group, control group[ (563.36 ±48.18) ng/L vs. (341.39 ±46. 23) ng/L, (330. 03 ±48. 06) ng/L,P 〈0. 01 ]. The serum leyel of MMP-9 was higher in AMI group than in UAP group [ ( 598. 88 ± 29. 34 ) ng/L vs. ( 526.95 ± 34. 56 ) ng/L, P 〈 0. 01 ]. No significantly statistics significance was found between SAP group and control group ( P 〉 0. 05 ). Conclusions The serum 25-hydroxy vitamin D and MMP-9 level was negatively correlated in patients with acute coronary syndrome. The serum 25-hydroxy vitamin D was reduced in ACS,its change may be related with the plaque inflammatory, and may play an important role in the athogenesis mechanism of ACS.
