摘  要:目的探讨超声检测高血压患者颈动脉重构与血流动力学变化的价值。方法采用彩色多普勒超声检测老年高血压患者(A组,200例)和非高血压体检老年人(B组,100例)颈动脉内-中膜厚度(IMT)、动脉内径、收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、血管阻力指数(RI)、血管搏动指数(PI)和平均流速(Vmean)。结果与B组比较,A组颈总动脉和颈内动脉内径狭窄、IMT增厚、斑块发生率高(P<0.05)。A组颈总动脉PSV、EDV和Vmean低于B组,RI、PI高于B组(P<0.05);A组颈内动脉的EDV和Vmean低于B组,RI高于B组(P<0.05)。结论老年高血压患者存在颈动脉重构及血流动力学变化。采用彩色多普勒超声检测能有助于早期发现患者颈动脉粥样硬化,对预防和治疗高血压血管损害具有重要临床价值。Objective To investigate the value of ultrasound examination of carotid reconstruction and hemodynamie changes in elderly patients with essential hypertension. Methods The carotid artery thickness (IMT), arterial diameter, peak systolic blood flow velocity (PSV), end-diastolic velocity (EDV), vascular resistance index (RI), pulsation index (PI), and mean flow velocity (Vmean) were examined with color Doppler ultrasound in 200 elderly patients with essential hypertension(group A) and 100 healthy elderly without hypertension(group B). Results Compared to group B, the diameter of common carotid artery and internal carotid artery was narrower, IMT was thicker,and incidence of plaque was higher in group A(P〈0. 05). The PSV, EDV and Vmean of common carotid artery were lower, RI and PI were higher, in group A than those in group B (P〈0.05). The EDV and Vmean of internal carotid artery were lower and RI was higher in group A than those in group B(P〈0. 05). Conclusion The elderly patients with essential hypertension are with carotid reconstruction and hemodynamic changes. The color Doppler ultrasound examination is helpful in early detecting carotid atherosclerosis, which is of value in the prevention and treatment of hypertension-induced vascular damage.
