明显脊柱关节症状的关节病型银屑病1例

摘　　要：患者男,38岁。双下肢红斑、斑块、鳞屑5年,双手关节疼痛1年,腰痛、晨僵半年。查体:脊柱活动受限,胸廓活动度下降。皮肤科情况:全身可见散在红斑、斑块,表面覆银白色鳞屑;左手指间关节红肿、活动受限,全部指甲增厚、变形。左手和腰椎X线分别提示银屑病性关节炎及强直性脊柱炎改变。HLA-B27阳性。诊断:关节病型银屑病;强直性脊柱炎。予环孢素胶囊治疗1个月,皮疹及关节疼痛缓解。A 38-year-old male presented with erythema, plaque and scale on both lower limbs for 5 years, arthralgia of both hands for 1 year and chronic pain and morning stiffness in the low back for half a year. Dermatological examination showed diffusive inflammatory erythematous plaques with silvery white scales on the trunk and extremities, stiff and swollen interphalangeal joints of the left hand, thickened and deformed nails. X-ray showed psoriatic arthritis in interphalangeal joints of the left hand and ankylosing spondylitis in lumbar vertebrae. The HLA-B27 was positive. Diagnosis： psoriasis arthropathica complicated with ankylosing spondyli- tis. Both lesions and joint pain were improved after treatment with Cyclosporine capsule for one month.
