聚丙烯酰胺水凝胶注射隆乳术后乳腺组织中NF-κ Bp65和TNF-α的表达及意义

摘　　要：目的:通过检测聚丙烯酰胺水凝胶(PAHG)置入人体后乳房病变组织内NF-κBp65、TNF-α表达,探讨二者与PAHG注射隆乳术后乳腺病理改变的关系。方法:采用免疫组化染色法检测20例正常乳腺组织,40例PAHG注射隆乳术后乳房病变组织中NF-κBp65、TNF-α阳性表达细胞,比较分析两组之间的差异。结果:①HE染色结果:光镜下见切片中含大量均质无定形凝胶样注射物,PAHG周围有不同程度纤维组织增生,伴或不伴纤维组织玻璃样变性,并见炎性细胞浸润;或局部呈异物巨细胞反应,部分乳腺导管扩张;②免疫组化结果:NF-κBp65、TNF-α阳性细胞少见于正常乳腺组织中,而在实验组近注射物周围NF-KBp65、TNF-α阳性细胞数目增多,具有显著统计学意义。但是NF-KBp65、TNF-α阳性细胞率与PAHG体内存留的时间长短无明显相关性。结论:NF-κBp65与TNF-α在PAHG注射隆乳术后病变组织中的表达存在正性相关关系。NF-κBp65、TNF-α参与了聚丙烯酰胺水凝胶注射隆乳者乳房病变组织的免疫反应过程,可能通过相互影响而共同诱导病变部位无菌性炎症、纤维组织增生等反应的发生。Objective To observe the rate of NF-KBp65 positive cells and TNF-α positive cells in human breast tissues with Polyacrylamide Hydrogel （PAHG） Injection.By contrasting with control group we should guess the relationship between NF-KBp65,TNF-α positive with immunoinflammation,fibroelastosis in human breast tissues with PAHG Injection.Methods Detect the expression of NF-KBp65,TNF-α in 40 cases with PAHG Injection and 20 normal breast tissues by immuno-histochemistry sp,and analyzed all the samples combining with HE staining.Results ① Masses of homogeneous amorphous polyacrylamide gel could be observed under light microscope.Around the PAHG,there were fibrous tissues proliferation with or without hyaline degeneration,and plenty of macrophages,monocytes,and histicytes but lymphocytes acuumulates surrounded.② Positive cell of NF-κBp65,TNF-α were hardly found in normal breast tissues,while lots of them could be observed in human breast lesion tissues Injected with PAHG,especially obvious in fibrous connective tissue area （besides the PAHG）.And the expression was not ratio with implantion time.Conclusion NF-κBp65 and TNF-α play an important part in immuo-inflammatary reactions of breast tissues with PAHG Injection.Which induce fibrous tissues proliferation and aseptic inflamation in lesion area.Meanwhile,NF-κBp65 and TNF-α has Pearson positive relevance in the changes.
