Dynesys动态稳定系统治疗腰椎退变性疾病的短期疗效分析

摘　　要：目的：探讨Dynesys动态稳定系统在腰椎退变性疾病治疗中的短期临床疗效。方法29例腰椎退变性疾病患者在后路减压的同时行Dynesys动态稳定系统内固定术，评价术后疗效。结果术后临床疗效优23例（80%）、良4例（14%），可2例（6%）。 VAS疼痛评分（0~10分）术前7.14±1.29分，末次随访时2.26±1.03分。术前病变脊柱运动节段过伸过屈活动度（ROM）7.51±2.68度，术后末次随访时ROM为4.22±1.47度。随访期间未见内固定松动和螺钉断裂现象。结论 Dynesys动态稳定内固定系统治疗腰椎退变性疾患具有手术操作简便、安全性高等优点，可减少对脊柱生理结构的破坏，维持节段稳定，减少远期腰椎退变和再次手术的风险，有很好的临床应用前景。Objective To analyze the short-term efficacy of the dynamic stabilization system in the treatment of lumbar degenerative disease. Methods We conducted posterior decompression and simultaneously internal fixation with dynamic stabilization system in 29 patients suffered from lumbar degenerative disease,and the curative effect was evaluated. Results The postoperative outcomes showed excellent in 23 cases （80%）,good in 4 cases （14%）and fair in 2 cases （6%）. VAS score was decreased from （7.14±1.29） points preoperation to （2.26±1.03）points at the last follow-up. The preoperative segmental range of motion （ROM） of the operative segments was （7.51±2.68）de-gree, and that was（4.22±1.47） degree at the last follow-up. There was no screw loose or rupture during the follow-up. Conclusion Dynamic stabilization system in the treatment of lumbar degen-erative disease has many advantages such as easy to operate, high safety,and so on,and it can reduce the destruction of the spine,maintain segmental stability,reduce long-term lumbar degenera-tion and the risk of reoperation. Dynamic stabilization system has good clinical application prospect.
