超声对膝上穿静脉瓣膜功能不全的评价作用

摘　　要：目的 通过超声在下肢静脉的应用,分析膝上功能不全穿静脉（IPV）解剖分布及其与临床分级关系、深浅静脉瓣膜反流程度的关系.方法 对61条膝上功能不全穿静脉进行回顾性分析.所有病例均进行了穿静脉结扎术,且术前均进行彩色多普勒超声检查.首先分析膝上IPV的解剖学特点,然后分析膝上IPV与临床分级关系,最后分析膝上IPV的临床分级与深浅静脉瓣膜功能不全、小腿IPV的关系.结果 膝上穿静脉主要分布在大腿中段内侧和大腿下段的内侧、后方,临床级别占比例较大为C3、C4,合并深浅静脉功能不全及膝下穿静脉功能不全的病例多出现在C5、C6.结论 超声是探查膝上穿静脉功能不全的可靠方法.膝上IPV多出现在大腿中段内侧、小腿下段内侧、后方;膝上IPV最常出现临床级别C2～C4,可以不伴有深静脉瓣膜功能不全,能独立致病;合并深、浅静脉反流和膝下穿静脉瓣膜功能不全时,临床级别增加.Objective The purpose of this study was the investigation of the distributionof incompetent perforating veins （IPVs） of the thigh segment in patients with different classes of chronic venous insufficiency （CVI） as defined by the updated clinical,etiologic,anatomic,and pathologic classification （CEAP） in relation to the pattern and the extent of venous reflux.and the relationship between deep venous valvular regurgitation,summarize clinical characteristics and practical clinical value in the thigh segment insufficiency perforating veins.Methods On the 61 lap insufficiency perforating veins were retrospectively analyzed.All patients were in vein ligation and preoperative color Doppler ultrasonography.First analyzes the anatomical characteristics of the thigh IPVS; and then analyzes the relation between the thigh IPVS and clinical grading; finally analyses the relationship between clinical classification and deep venous valve function of the thigh IPVS insufficiency and the calf IPV.Results The ultrasound and operation found insufficiency perforating veins similar amount,the thigh IPVS are mainly distributed in the middle and lower section of thigh. clinical level accounted for a larger proportion of C3,C4,combined with cases of perforating venous insufficiency of deep venous or the calf IPVs insufficiency appear in the C5,C6.Conclusions Ultrasound diagnosis of the thigh IPVsand operation results are consistent,the thigh IPVS appears to have differences in the clinical level,Clinical often appear for the moderate level,can not be combined with deep venous valve insufficiency,can independent risk,when combined with deep venous reflux and the calf IPVs,clinical level increase.
