蔗糖铁联合左卡尼汀对维持性血液透析伴慢性肾性贫血患者的疗效及安全性

摘　　要：目的探讨蔗糖铁联合左卡尼汀对维持性血液透析伴慢性肾性贫血的效果及其不良反应。方法选取2010年4月至2013年4月我院接收治疗的血液透析尿毒症合并慢性肾性贫血患者120例,按照就诊时间均分为A、B、C 3组,每组40例,三组患者均给予贫血基础治疗。A组不作其他任何处理,作为对照组;B组给予蔗糖铁治疗;C组给予蔗糖铁联合左卡尼汀治疗。在治疗后8、16周检测三组患者的血红蛋白、血清铁蛋白、红细胞比容、血清白蛋白浓度、转铁蛋白饱和度以及治疗24周后各组患者治疗有效率、不良反应发生率。结果治疗8周时,C组患者上述各指标含量高于A、B两组,但差异无统计学意义(P>0.05);治疗16周后,C组患者各指标均显著高于A、B两组(P<0.05),治疗24周之后,治疗有效率C组显著高于A、B两组(P<0.05);不良反应发生率C组显著低于A、B两组(P<0.05)。结论蔗糖铁联合左卡尼汀对血液透析尿毒症合并慢性肾性贫血患者疗效显著,值得临床推广使用。Objective To discuss the efficacy and adverse reactions of iron sucrose combined with levocarnitine on maintenance hemodialysis complicated with chronic kidney anemia. Methods 120 cases of patients with maintenance hemodialysis complicated with chronic kidney anemia in our hospital from April 2010 to April 2013 were selected and divided into A,B,C groups,40 cases in each group,basic therapy for anemia was adopted in three groups.Group A was selected as the control group without any other treatments,group B was treated with iron sucrose,group C was treated with combined iron sucrose levocarnitine. After treating for 8 weeks,16 weeks,concentration of hemoglobin,serum ferritin（SF）,hematocrit,serum albumin,transferrin saturation and significant rate,incidence of adverse reactions 24 weeks after treatment were detected. Results 8 weeks after treatment,all indicators of group C were higher than those of group A and group B,but had no significant difference（P ＆gt; 0. 05）;16 weeks after treatment,all indicators of group C were significantly higher than those of group A and group B（P ＆lt; 0. 05）;24 weeks after treatment,significant rate of treatment of group C was significantly higher than those of group A and group B（P ＆lt; 0. 05）;incidence of adverse reactions of group C was significantly lower than those of group A and group B（P ＆lt; 0. 05）. Conclusion The efficacy of iron sucrose combined with levocarnitine on maintenance hemodialysis complicated with chronic kidney anemia is significant,which deserves to be promoted and applied in clinic.
