中西医结合治疗慢性骨髓炎的临床疗效

摘　　要：目的：观察中西医结合治疗慢性骨髓炎的疗效。方法本研究为前瞻性研究，即2004年01月-2010年02月笔者所在医院收治的共121例慢性骨髓炎患者随机分为3组进行治疗。中医治疗组应用参芪扶正汤内服，骨疽易消散外用（以上中药均为院内经验方剂）；西医治疗组应用手术清除死骨及窦道，术后持续灌流冲洗2～3周，常规换药，全身应用敏感抗生素；中西医结合组在全身应用抗生素和清除死骨、窦道的基础上给予参芪扶正汤内服，术后外用骨疽易消散治疗。结果121例均获随访，时间8～56个月，平均30个月。中医治疗组总有效率达66．7％，西医治疗组总有效率54．8％，中西医结合组总有效率88．9％；中医治疗组平均住院治疗时间59．3 d，西医治疗组平均住院治疗时间74．6 d，中西医结合组平均住院治疗时间43．7 d。中西医结合组疗效、平均住院治疗时间与中医治疗组、西医治疗组比较有显著性差异（P＜0．01）。结论中西医结合治疗慢性骨髓炎疗效优于中医或西医单纯治疗，且缩短治疗时间。Objective To investigate the clinical effect of chronic osteomyelitis treated with integrated therapy of Chinese and Western medicine. Methods This study is a prospective one. A total of 121 cases of chronic osteomyelitis（from January 2002 to February 2010） were divided into three groups：respectively treated with methods of traditional Chinese medicine （TCM）, methods of Western medicine and integrated therapy of both. Results All the patients were followed up （ranged from 8 to 56 months）,with average of 30 months. The total effective rate of cases treated with methods of TCM was 66.7%,that of cases treated with methods of Western medicine 54.8%,that of cases treated with integrated therapy 88.9%;The average period of hospitalization of cases treated with methods of TCM was 59.3 days,that of cases treated with methods of Western medicine 74.6 days, that of cases treated with integrated therapy 43.7 days. There was significant difference among three groups in total effective rate and average period of hospitalization （P＆lt;0.01）. Conclusion The effect of chronic osteomyelitis treated with integrated therapy is superior to alone TCM or with Western medicine ,and its treatment time could be shortened.
