摘 要:目的:观察北美Draeger GS麻醉机吸气流速装置对呼吸功能的影响.方法:随机选择100例静吸复合全麻下的手术患者,其中,男性51例,女性49例;年龄为17~85岁;体质量为46~95 kg;甲状腺、乳腺手术29例,泌尿外科手术71例.应用北美GS麻醉机机械通气,吸气流速可变装置先后在Low挡、Med挡位各机械通气30 min后,分别行动脉血气分析,观察通气参数(Vt)、气道阻力(Peak).比较吸气流速Low、Med挡位时机械通气动脉血气、Vt、Peak的差异.结果:100例临床观察结果显示,吸气流速调至Med挡后,较Low挡时PaCO2、PetCO2有显著下降(P<0.05),同时Vt、Peak则有显著上升(P<0.05),但pH、p(O2)变化则无统计学意义(P>0.05).结论:血气是呼吸功能的最终标志.在保持机械通气参数不变的情况下,增加呼吸器吸气流速,有利于吸入气在不同肺泡区间的均匀分布,改善通气效果,但胸内压的相应增加,导致的Peak增高,应予注意.Objective To observe the effect of inspiratory flow device in North American Draeger GS anesthesia machine on respiratory function.Methods Totally 100 cases undergoing surgery were randomly selected under general anesthesia,with 51 cases of male and 49 cases of female,age ranging from 17 to 85 years old,and weight from 46 to 95 kg.There were 29 cases receiving thyroid or breast surgery,and 71 cases receiving urological surgery.North American Draeger GS anesthesia machine was applied in all these cases for mechanical ventilation (Vt:from 7 to 9 mL/kg,F:from 10 to 12 times/min,I/E:1/2),Inspiratory flow device changed to Low and Med status during each mechanical ventilation.After 30 min,arterial blood gas analysis was performed,meanwhile Vt and Peak were observed for comparison.Results Turning inspiratory flow device to Med status,PaCO2 and PetCO2 decreased significantly (P〈0.05),while Vt,PEAK was significantly increased (P〈 0.05),but there were no significant differences between pH and p(O2) (P〉0.05) compared with Low status.Conclusion Blood analysis is the ultimate sign of respiratory function.This clinical observation shows that,while mechanical ventilation parameters do not change,increasing ventilator inspiratory flow in North American Draeger GS anesthesia machine can reduce physiological dead space volume,and improve ventilation effect.However,intrathoracic pressure will increase spontaneously,leading to high Peak.
