摘 要:目的观察螺内酯对糖尿病(DM)大鼠心肌组织第10号染色体缺失的磷酸酶和张力蛋白同源基因(PTEN)表达的影响,探讨螺内酯治疗糖尿病心肌病(DCM)的作用及机制。方法雄性SD大鼠36只,随机分成正常对照(NC)组、糖尿病(DM)组和糖尿病螺内酯治疗(DS)组,每组各12只。采用STZ一次性腹腔注射复制DM动物模型,DS组给予螺内酯40 mg/(kg·d)灌胃,NC组和DM组每日给予等量生理盐水灌胃,8周后观察体质量及血清学指标,采用免疫组织化学染色检测心肌组织PTEN、p-Akt(Ser473)蛋白的表达,采用实时定量RT-PCR检测心肌组织PTEN mRNA的表达。结果 DM组和DS组TG、TC、FBG、GHbA1c水平均高于NC组(P<0.01),而两组间差异无统计学意义,3组之间血钾水平差异无统计学意义;DM组大鼠出现心肌肥大而DS组心肌病变明显减轻;DM组PTEN mRNA和蛋白的表达明显低于NC组(P<0.01),DS组则高于DM组(P<0.05);DM组p-Akt(Ser473)蛋白表达明显高于NC组(P<0.01),DS组则低于DM组(P<0.05)。结论螺内酯能通过上调心肌组织PTEN表达,抑制AKT激活,减轻DM大鼠心肌肥大。【Objective】 To explore the mechanisms of Spironolactone on diabetic cardiomyopathy(DCM) by observing the effects of Spironolactone on the PTEN expression in myocardial tissues of diabetic rats. 【Methods】36 male SD rats were randomly divided into 3 groups which include normal control group(group NC, n =12), diabetic group(group DC, n =12) and Spironolactone group(group DS, n =12). The diabetic rat models were established by intra-peritoneal injection with Streptozotocin while group DS received Spironolactone 40mg.kg·d and group NC and group DM were given normal saline by gavage, body weight and serum markers were collected 8 weeks later, in which immunohistochemistry method was adopted to detect the protein expression of PTEN, p-Akt(Ser473), real-time PCR was used to detect the mRNA expression of PTEN in myocardial tissue. 【Results】 TG, TC, FBG, GHbA1c levels in group DC and group DS were higher than those in group NC(P〈0.01) and there was no significant difference between group DC and group DS, serum potassium levels in the three groups showed no significant difference; the diabetic cardiomyopathy of group DS were significantly improved compared with group DM; the PTEN mRNA and protein expression, compared with group NC, were significantly lower in group DM(P〈0.01) and the one in group DS is higher than that in group DM(P〈0.05); compared with group NC, the p-Akt(Ser473) protein expression was significantly higher in group DM(P〈0.01) and the one in group DS is lower than that in group DM(P〈0.05). 【Conclusion】 Spironolactone may prevent myocardial hypertrophy by increasing PTEN and decreasing Akt levels in diabetic rats.
