口腔扁平苔藓病损区细胞凋亡蛋白Bcl-2、Bax的表达研究

摘　　要：目的通过对口腔扁平苔藓病变发展过程中细胞凋亡状况和凋亡相关蛋白表达的研究,探讨扁平苔藓的发病机制。方法采用免疫组化SP法,分析正常口腔黏膜上皮25例,扁平苔藓组织25例中凋亡相关蛋白Bcl-2、Bax表达水平及分布差异。结果与正常黏膜比较,Bcl-2在口扁平苔藓固有层与上皮层均显过度表达(P<0.05),随淋巴细胞聚集密度的增加,其在固有层表达强度也明显增强,并高于上皮层(P<0.05)。正常黏膜Bax弱表达局限于角化层,口腔扁平苔藓中Bax表达较正常组明显增强,随糜烂程度的加重,Bax表达明显增强(P<0.01),分布更广泛。结论 Bcl-2在扁平苔藓固有层中过度表达抑制了淋巴细胞的凋亡的,强化细胞免疫,Bax过度表达是对异常增殖的上皮细胞和免疫细胞的拮抗反应,试图通过诱导凋亡,恢复细胞数目的稳定和免疫的平衡,但是过度凋亡致糜烂萎缩皮损。Objective To explore the pathogenesis of oral lichen planus （ OLP） through examining the expression of apoptosis-associated protein Bcl-2 and Bax in OLP .Methods The expression levels and distribution of Bcl-2 and Bax were detected by immunohistochemistry（SP） in 25 cases of normal mucosa and 25 cases OLP tissues.Results As compared with that in normal mucosa,overexpression of Bcl-2 was found in lamina propria and epithelial lining of OLP （ P 〈0.05）,the expression levels of Bcl-2 in lamina propria of OLP were increased with the increase of lymphocyte aggregation dencity ,which were significantly higher than those in epithelial lining of OLP （ P 〈0.05）.The weak expression of Bax in normal mucosa was limited in cuticular layer , however , the expression levels of Bax were increased , as compared with those in normal mucosa , moreover,which were further increased with the aggravation of erosion degree （ P 〈0.01）,and its distribution was more wide. Conclusion The overexpression of Bcl-2 in lamina propria of OLP inhibits apoptosis of lymphocytes and strengthens cellular immunity .The overexpression of Bax is antagonism reaction to epithelial cells and immunocyte of abnormity proliferation ,which try to recover stabilization of cell number and immune balance by inducing cell apoptosis ,however,excess apoptosis can result in erosion,atrophy,skin injury in OLP.
