摘 要:目的探讨连续性照护的疾病管理模式对输尿管腹壁造口患者的影响。方法将60例行膀胱全切输尿管腹壁造口的膀胱癌患者按随机数字表法分为观察组和对照组各30例。对照组采用常规护理;观察组由泌尿外科尿路造口专科护士给予全程的连续性照护管理,包括入院时建立个人疾病档案,制定系统、全程、规范的个性化健康教育档案,实施全程监测、干预。结果观察组出院后不同时间造口知识掌握情况显著高于对照组(均P<0.01),负性情绪及并发症发生率显著低于对照组(P<0.05,P<0.01)。结论对输尿管腹壁尿路造口的膀胱癌患者实施连续性照护,可以有效提高患者对疾病的认知,减少负性情绪的产生。Objective To explore the effect of continuous care of bladder cancer patients with a stoma created in the abdominal wall after ureterostomy. Methods Sixty bladder cancer patients after total resection of bladder and ureterostomy were equally divided in- to an observation group and a control group randomly. The control group was given conventional care, while the observation group was given continuous care by astomy nurses, including building disease archives on admission, setting up systematic, whole-course and normative individualized health education files, as well as whole-course monitoring and intervention. Results The observation group were significantly more knowledgeable about stoma care than the control group(P〈0.01 for all), while rate of negative emotion episodes and complications of the observation group were significantly lower than those of the control group(P〈0.05 ,P〈0 0.01). Conclusion The continuity of care of bladder cancer patients after total resection of bladder and ureteral stoma creation can effectively improve their cognition of the disease and reduce the episodes of negative emotions.
